摘  要:目的研究上颌中空赝复体患者卫生指导前后不同时期龈下牙周菌群的动态演变过程,探讨卫生指导对牙周微生态的影响。方法试验组为10例赝复体门诊就诊患者,对照组为10名未行任何修复的牙周健康者。对2组卫生指导前后龈下可培养细菌菌落数及革兰氏阴性菌构成比进行检测和比较。结果试验组和对照组卫生指导前每毫升送检液中菌落数经对数转换后的值分别为1.070 4±0.038 6、0.992 8±0.052 5,指导后3个月分别为0.946 8±0.050 4、0.888 0±0.062 7;试验组和对照组卫生指导前革兰氏阴性菌构成比分别为(55.81±5.21)%、(50.08±7.07)%,指导后3个月分别为(41.56±7.62)%、(38.21±7.58)%。2组卫生指导后,龈下菌落数和革兰氏阴性菌构成比均有减少(P<0.05);试验组卫生指导前后,龈下菌落数和革兰氏阴性菌构成比均高于对照组(P<0.05)。结论进行卫生指导后,赝复体患者口腔菌群的生存状态有了改善。口腔卫生指导能有效减少赝复体患者患牙周病的机率。Objective To study the dynamic changes of subgingival periodontal microflora of the different periods, and to explore the impact of oral hygiene instruction on periodontal microflora in patients with maxillary hollow prosthesis. Methods Test group consisted of ten patients with maxillary hollow prosthesis. And we selected ten periodontal healthy people without any prosthesis as control group. The total number of bacterial colonies and G-bacteria constituent ratio of the gingival sulcus of the abutments were detected,and the differences of them before and after oral hygiene instruction in the two groups were compared. Results Before oral hygiene instruction,the total amount of subgingival culturable bacterial colonies in trial group and control group were 1. 070 4 ± 0. 038 6 and 0. 992 8 ± 0. 052 5 after logarithmic transformation,respectively. And there months after oral hygiene instruction,the bacterial colonies amount were 0. 946 8 ± 0. 050 4 in trial group and 0. 888 0 ± 0. 062 7 in control group. The G-bacteria constituent ratio in trial group changes from( 55. 81 ±5. 21) % before oral hygiene instruction to( 41.56 ±7.62) % after instruction,and that of control group were( 50. 08 ±7. 07) % and( 38. 21 ±7. 58) % before and after instruction,respectively. The number of subgingival bacterial colonies and G-bacteria constituent ratio were lower than that before oral hygiene guidance in both groups( P 〈 0. 05); For the trial group,the number of subgingival bacterial colonies and G-bacteria constituent ratio were higher than that of control group,before and after instruction. Conclusion After oral hygiene instruction,the oral flora survival state of the patients with prosthesis has improved. Oral health guidance can effectively reduce the risk of prosthetic patients suffering from periodontal disease.
