妊娠合并地中海贫血与弓形体病相关性的临床分析

摘　　要：目的:探讨妊娠合并地中海贫血与弓形体(TOX)感染之间的相关性。方法:收集1 247例孕妇血液,采用红细胞脆性试验"一管法"进行地中海贫血的初步筛查并对可疑贫血及地中海贫血病例再进行基因分型;利用聚合酶链反应法检测TOX DNA阳性率,同时用五分类血球计数仪测定血红蛋白(Hb)和平均红细胞体积(MCV)并进行比较。结果:1 247例孕妇中地中海贫血118例,阳性率为9.46%;TOX感染159例,阳性率为12.75%;地中海贫血与TOX合并感染者96例,阳性率为7.70%,地中海贫血合并TOX感染之间存在显著性相关。在进一步对Hb、平均红细胞体积(MCV),谷丙转氨酶(ALT)、谷草转氨酶(AST)、谷氨酰转肽酶(GGT)检测中发现孕妇地中海贫血合并TOX感染组分别与单纯地中海贫血或单纯TOX感染组比较,两组之间差异有统计学意义(P<0.05)。结论:妊娠合并地中海贫血的血液中存在着TOX合并感染,且可能与地中海贫血的发展密切相关。Objective： To explore the correlation between pregnancy combined with Mediterranean anemia and toxoplasma（ TOX）infection. Methods： Blood specimens of 1 247 pregnant women were collected,erythrocyte fragility test ＂ a tube method＂ was used for primary screening of Mediterranean anemia,genotyping was performed among the women suspected of anemia and Mediterranean anemia; PCR was used to detect the positive rate of TOX DNA,five classification globulimeter was used to detect hemoglobin（ Hb） and mean corpuscular volume（ MCV）,then the results were compared. Results： Among 1 247 pregnant women,118 women were diagnosed as Mediterranean anemia,the positive rate was 9. 46%; 159 women were found with TOX infection,the positive rate was 12. 75%; 96 women were diagnosed as Mediterranean anemia combined with TOX infection,the positive rate was 7. 70%. There were statistically significant differences in Hb,MCV,alanine transaminase（ ALT）,aspartate aminotransferase（ AST） and gamma- glutamyl transpeptidase（ GGT） levels between the women with Mediterranean anemia combined with TOX infection and the women with simple Mediterranean anemia or simple TOX infection（ P〈0. 05）. Conclusion： TOX infection in patients with Mediterranean anemia may be closely correlated with development of Mediterranean anemia.
