血管内皮生长因子在脑梗死患者外周血中表达的初步研究

摘　　要：目的探讨血管内皮生长因子(VEGF)在脑梗死外周血中的表达情况及丁苯酞注射液对VEGF的作用。方法应用ELISA法检测60例急性脑梗死患者(脑梗死组)发病1、3、7、10、14d和50例健康体检者(对照组)血清VEGF水平,头颅CT测量梗死灶面积,将中等面积脑梗死患者随机分成用药组和非用药组,非用药组按常规治疗方案,用药组在病后第一天至第十四天加用丁苯酞注射液,2次/d 25mg静滴,3个月后采用mRS评分评估预后。结果急性脑梗死患者各时相血清VEGF水平均高于对照组(P<0.01);VEGF水平发病当天即升高,在7d达最高,第十天开始下降,14d降至最低;用药组患者血清VEGF水平高于非用药组(P<0.01);预后较好患者血清VEGF水平高于预后较差者(P<0.01)。结论丁苯酞注射液能使VEGF表达增强,可能在一定程度上改善急性脑梗死患者的预后。Objective To observe the dynamic changes of serum VEGF in the different time points in pa- tients with cerebral infarction and the role butylphthalide injection on VEGF expression. Methods Sixty cases were detected by ELISA in patients with acute cerebral infarction {CI group） onset ld, 3d, 7d, 10d, 14d and 50 healthy subjects {control group） in serum levels of VEGF. According to the area of the infarct by CT measurement, the patients with middle cerebral infarction were randomly divided into the experimen- tal group and the control group, the control group received conventional therapy, while the experimental group was plus SBR peptide injection, 25mg twice daily intravenous from the first day to 14th days after disease, then assessed the prognosis with mRS score 3 months later. Results Compared with control group, the CI group at each time point serum VEGF levels were significantly increased （P〈0.01）. VEGF levels showed dynamic changes, began to rise namely that day disease, at 7 d up to the highest, the tenth day began to decline, at 14 d to a minimum. Serum VEGF levels in patients of experimental group than in the control group （P〈0.01）. Better prognosis in patients with higher serum VEGF levels with poor prog- nosis （P〈0.01）. Conclusion SBR peptide allows the expression of VEGF increased, may improve the prognosis of patients with acute cerebral infareti on in a certain extent.
