摘 要:目的比较同患者恶性胸腔积液上清、白细胞层及离心细胞块三种不同成分DNA提取质量,探索最有效的胸腔积液成分用于DNA提取及方法。方法收集解放军第一七五医院2014年细胞学及细胞块明确诊断恶性新鲜胸腔积液5例,分别用胸腔积液上清、白细胞层及离心细胞块三种不同成分进行DNA提取,然后对DNA含量和纯度进行测定,比较其DNA提取效果。结果对同一患者胸腔积液中白细胞层提取DNA含量最高,纯度次之,但对后续的EGFR基因突变和测序状况无影响;细胞块DNA提取质量次之,但纯度最高;胸腔积液上清提取质量及纯度均最低。结论恶性胸腔积液DNA提取用于EGFR基因突变检测的最佳成份为离心白细胞层,离心细胞块次之,本实验组胸腔积液上清提取效率不高。Objective To compare the quality of different components of DNA which were malignant pleural effusion, leukocytic cream and pleural fluid cell blocks extracted from malignant pleural effusion, and to explore the most effective ingredients for pleural fluid DNA extraction. Methods Five cases of pleural effusion were collected from 175th Hospital of PLA at the period of 2014 after distinguish diagnosis by cell smear and conventional differential diagnosis of cell block, then DNA of different components which were malignant hydrothorax supernatant, leukocytic cream and pleural fluid cell blocks were extracted respec- tively. The content and purity of DNA were determined, and the effect of extraction of DNA was compared. Results The content of DNA extracted from leukocytic cream of the same patients was the highest, purity was the second, played nothing important role on subsequent analysis of EGFR gene mutation and sequencing. Followed by the content of DNA extracted from pleural fluid cell blocks occupied the second place, but the highest purity. The content of DNA extracted from malignant pleural effusion were the lowest. Conclusion The optimum component of malignant pleural fluid DNA to detect EGFR gene mutations is the leukocytic cream, pleural fluid cell blocks and hydrothorax supernatant in sequence according to the extraction rate of DNA.
