摘　　要：目的探讨小瞳孔下应用弹性虹膜拉钩进行晶状体超声乳化联合人工晶状体植入术的安全性和临床效果。方法30例（30眼）小瞳孔白内障，术中应用弹性虹膜拉钩扩大瞳孔，采用透明角膜隧道切口行超声乳化吸出联合人工晶状体植入术，术后随访1～6个月。结果术后随访，26眼视力均较术前明显提高，4眼因眼底病视力未提高。所有患者术后眼压维持正常。手术并发症主要有角膜水肿、前房纤维素性渗出及后囊膜破裂等。结论小瞳孔下白内障超声乳化手术中采取弹性虹膜拉钩扩大瞳孔安全方便，可以降低手术风险，提高视力，达到较好的手术效果。Objective To investigate the safety and efficacy of iris hook in the phacoemulsification on eyes with small pupil. Methods Thirty eyes of 30 patients with small pupil and cataract underwent phacoemulsification combined with foldable intraocular lens implantation were enrolled in this study. Iris hooks were applied to dilate the pupil in the operation. The patients were followed up for 1 to 6 months. Results During the postoperative follow-up examination, the visual acuities of 26 patients were improved compared with preoperative levels with normal intraocular pressure （ IOP ） and visual acuity of 4 patients remained unchanged because of fundus diseases. The surgical complications included corneal edema, anterior chamber fibrin exudation and posterior capsular rupture. Conclusion Application of iris hooks in the phacoemulsifieation operation of eyes with small pupil is safe and convenient. It can reduce the risk of surgery, improve the visual acuity and achieve better surgical effect.
