摘 要:目的探讨急性白血病五色流式细胞术免疫分型与FAB分型的关系和表达规律。方法通过CD45/SSC设门的五色流式细胞仪检测118例急性白血病患者骨髓细胞的表面抗原,并进行免疫表型分析。结果在88例AML患者中髓系相关抗原的表达率依次为CD33(93.2%)>CD13(88.6%)>CD117(86.4%),部分AML患者伴表达淋系相关抗原,主要为CD56(25.2%)、CD7(11.3%)、CD19(2.2%)、CD20(2.2%);M3中高表达CD117(83.7%)、CD13(83.7%)、CD33(83.7%),但均不表达CD34、HLA-DR,且SSC较大;M4、M5中可见CD64、CD4、CD14明显表达高于AML其他亚型(P<0.01)。19例B-ALL患者中CD19的表达率最高(100%),其次为CD34(90.1%)、HLA-DR(90.1%)、CD10(90.1%);9例T-ALL患者中CD7的表达率最高(100%),其次为CD4(77.8%)、CD5(77.8%)、CD2(66.7%)、CD3(66.7%)、CD34(55.6%)。结论通过五色流式细胞术进行白血病免疫分型是形态学分型诊断重要的补充,能够提供更客观的诊断依据,对提高急性白血病的诊断正确率、预后判断和治疗方案的选择有重要的意义。Objective To study the relationship between the immunophenotype by five-color flow cytometry and FAB phenotype in Acute leukemia. Methods Immunophenotyping of 118 cases of bone marrow specimens from patients with acute leukemia were analyzed by five-color FCM with CD45/SSC double parameters and scatter spot picture. Results In 88 cases of acute myeloid leukemia(AML), the myeloid-associated antigens were CD33(93.2%), CD13(88.6%), and CD117(11.3%), respectively, and the lymphoid-associated antigens were CD56(25.2%), CD7(11.3%), CD19(2.2%) and CD20(2.2%), respectively. The antigens mainly expressed in M3 were CD117(83.7%), CD13(83.7%), CD33(83.7%), and bigger SSC, but not CD34 and HLA-DR. The antigens mainly expressed in M4 and M5 were CD64, CD4 and CD14(P〈0.01). In 19 cases of patients with B-ALL, the expression rate of CD19 was the highest(100%), followed by CD34(90.1%), HLA-DR(90.1%) and CD10(90.1%). In 9 cases of patients with T-ALL, the expression rate of CD7 was the highest(100%), followed by CD4(77.8%), CD5(77.8%), CD2(66.7%), CD3(66.7%) and CD34(55.6%). Conclusion Based on a more objective diagnostic basis, five-color flow cytometry is extremely important in improving the diagnostic accuracy, therapy and prognosis in acute leukemia.
