慢性乙肝患者肝纤维化与肝硬度、超声量化指标、血清肝纤维化指标的相关性

摘　　要：目的探讨肝硬度检测、超声量化指标、血清肝纤维化指标在乙肝肝纤维化患者中的相关性。方法选择2010-03-2013-03我院56例慢性乙肝患者,进行肝穿刺病理组织检测、肝硬度检测、超声、血清透明酯酸（HA）与Ⅳ型胶原（CⅣ）检测,根据肝组织病理检测结果将肝纤维化患者肝纤维化程度由轻到重分为S0、S1、S2、S3、S4共五组,分析各组间的差异,然后进行肝硬度检测与超声量化指标、血清肝纤维化检测相关性分析。结果慢乙肝患者的肝硬度值、肝脏B超评分、HA与CⅣ随着肝纤维化病理分期的增加而增大,并且肝脏硬度值与肝脏B超评分、HA、CⅣ呈正相关（r分别为0.720,0.774,0.727,P均〈0.05）。结论慢性乙肝患者肝组织病理学检测、肝硬度检测、B超、血清肝纤维化指标检测具有一致性,各有优缺点,在肝纤维化诊断方面需综合判断,以提高肝纤维化诊断的准确性,更好地指导临床的预防和治疗。Objective To explore the relationship between the liver stiffness score,ultrasorLnd score and serum indices of liver fibrosis in chronic hepatitis B（CHB）patients. Methods Sixty-five CHB patients in our hospital from March 2010 to March 2013 were en- rolled in this study. FibroSean methods were applied to measure the liver stiffness score of all patients. Ulastography was applied to ob- serve the liver morphology. The levels of HA and C Ivwere determined. Liver biopsy was performed at the same period. All cases were divided into five groups（ SO, S1, S2, S3 and S4）according to the results of liver pathological biopsy. The correlations between the liver stiffness value, ultrasound score and serum indices of liver fibrosis were analyzed. Results The liver stiffness score, ultrasound score, the levels of HA and C IV in chronic hepatitis B patients increased as the liver biopsy stage increased. Liver stiffness was positively correlated with ultrasound score, HA,and CIV（r =0. 720,0.774,0. 727,P 〈0.05）. Conclusion Liver biopsy,liver stiffness score, ulastography and the indices of serum liver fibrosis are consistent in diagnosis of liver fibrosis. They have their own advantages and disadvantages, and should be compresively used to improve the accuracy of diagnosis of liver fibrosis.
