肝动脉置管泵化疗预防结肠癌术后肝转移的价值

摘　　要：目的:评估结肠癌根治术后肝动脉置管泵化疗预防结肠癌肝转移价值。方法:将33例按手术治疗后无肝、肺、脑等远隔器官转移的结肠癌患者随机数字表法分为治疗组和对照组,治疗组术后行肝动脉置管泵化疗,对照组行经外周静脉穿刺中心静脉置管化疗,比较两组患者无肝转移生存时间。结果:治疗组平均无肝转移生存期为(24.2±4.3)个月,对照组平均无肝转移生存期为(16.6±2.0)个月,治疗组平均无肝转移生存期明显高于对照组,比较差异有统计学意义(t=4.562,P=0.041);以肝转移为研究的终点事件,绘制两组的生存曲线,两组生存曲线比较差异具有统计学意义(Log-Rank字2=25.085,P=0.000)。结论:相比常规经静脉化疗,结肠癌根治术后经肝动脉置管泵化疗能有效延缓结肠癌根治术后肝转移,是预防结肠癌肝转移的有效方法。Objective： To evaluate the value of hepatic artery infusion pump chemotherapy for the prevention of liver metastasis after radical resection of colon cancer. Method： According to the different treatment methods, 33 cases without liver, lung, brain metastasis of distant organs in patients with colon cancer were randomly divided into treatment group and control group, the treatment group was treated with chemotherapy by hepatic artery infusion pump, while the control group underwent peripherally inserted central catheter chemotherapy, and the survival time without liver metastasis was compared. Result： In the treatment group, the average survival time without liver metastasis was（24.2±4.3） months, while in the control group, the average survival time without liver metastasis was（16.6±2.0） months. There were statistically significant differences between the two groups. The survival curves of the two groups by the endpoint of liver metastasis had statistically significant difference（Log-Rank χ^2=25.085,P=0.000）. Conclusion： Compared with the conventional intravenous chemotherapy, hepatic artery infusion pump chemotherapy after radical resection of colon cancer can effectively delay the liver metastasis after radical resection of colorectal cancer, and is a modus operandi for the prevention of liver metastases from colorectal cancer.
