手术治疗双额叶挫裂伤的疗效分析

摘　　要：目的探讨双额叶脑挫裂伤所致中心疝的临床特点,以及手术时机干预对患者预后的差别分析。方法回顾性分析2000年至2012年间84例双额叶脑挫裂伤所致脑中心疝患者的临床症状,并依据Plum分期将患者的临床表现与手术治疗后的疗效相结合,判断手术时机的选择对患者预后的影响。结果早期手术者预后较好者为85.7%,晚期手术者为6.6%,两组比较有统计学意义。结论双额叶挫裂伤患者从起病到中心疝的早期临床表现多较轻微,易被家属及医护人员忽视。脑疝的早期确诊及手术时机的判定直接影响患者的预后。经研究发现,早期确诊及选择手术治疗的患者较中晚期确诊手术的患者预后好(P<0.05),有统计学意义,通过我们的研究希望能提高临床医师对于中心疝的认识,对双额叶脑挫裂伤的患者的治疗起到积极作用。Objective To investigate the clinical characteristics of bilateral frontalcontusion and laceration of brain center hernia caused by theoperation, and the timing of intervention on prognosis of patients with difference analysis.Method A retrospective analysis from 2000 to 2012 the clinical symptoms of 84 cases of bilateral frontal brain contusion and laceration caused by brain center hernia patients, according to the Plum staging and the effect of clinical manifestations and surgical treatment in patients with the combination to judge the impact of timing of surgery on the prognosis of the patients.Results Early operation beter prognosis was 85.7%, advanced operation was 6.6%, two groups had statistical signiifcance.Conclusions Double frontal contusion and laceration patients from the onset to central hernia early clinical manifestation was slight, family members and medical staff is easy to be ignored. Early diagnosis and operation timing of cerebral hernia decision directly affects the prognosis of patients. The study found that early diagnosis and choice of surgical treatment in patients with more advanced diagnosis surgery prognosis of patients with good（P＆lt;0.05）, there is statistical signiifcance, through our research hope to improve the cognition of clinicians on central hernia, pair of frontal lobe contusion laceration patients treatment play a positive role.
