特发性眼眶炎性假瘤患者血清细胞间黏附分子-1水平及其临床意义

摘　　要：目的探讨特发性眼眶炎性假瘤（idiopathic orbital inflammatory pseudotumor,IOIP）患者血清细胞间黏附分子-1（intercellular adhesion molecule 1,ICAM-1）水平与IOIP的发生发展的关系。方法应用酶联免疫双抗体夹心法（ELISA法）测定60例IOIP患者（病例组）和30例正常健康者的血清ICAM-1水平。所有IOIP患者均经过病理组织学检查证实。采用SPSS 17.0软件比较IOIP组和对照组之间血清ICAM-1水平的差异,并分析IOIP组血清ICAM-1水平与病例及眼别的关系。结果 IOIP患者血清ICAM-1水平为（251.68±76.16）μg·L^-1,比正常对照组（176.56±29.80）μg·L^-1明显增高（P〈0.05）;随着IOIP病程的延长,ICAM-1的表达呈明显上升趋势,病程〉12个月患者（326.60±38.71）μg·L^-1与病程6-12个月患者（264.53±34.90）μg·L^-1及病程〈6个月患者血清ICAM-1水平（184.04±53.05）μg·L^-1两两比较差异均有统计学意义（均为P〈0.05）,双眼发病者血清ICAM-1水平（293.30±69.34）μg·L^-1明显高于单眼发病者（240.17±74.56）μg·L^-1（P〈0.05）。结论：OIP患者血清ICAM-1水平可反映病程长短和严重程度,检测血清ICAM-1水平在评估疾病的严重性、病程长短等方面具有一定的参考价值。Objective To investigate the level of serum intercellular adhesion molecule-1 （ ICAM-1 ） in idiopathic orbital inflammatory pseudotumor （IOIP） and its relationship with IOIP development. Methods The serum ICAM-1 level of 60 cases of IOIP and 30 cases of normal people were tested by ELISA. All cases of IOIP were diagnosed and confirmed by histopathologic examination. The difference of serum ICAM-1 expression between IOIP group and normal control group were compared by SPSS 17.0 software,and the relationship between serum ICAM-1 expression and duration of disease, eye laterality in IOIP group were analyzed. Results Serum ICAM-1 level of IOIP group and normal control group were （251.68 ± 76.16）μg·L^-1 and （176.56 ± 29.80） μg·L^-1, respectively, there was statistical difference （P 〈 0.05 ）. With the increase of the duration,there were dramatically increasing trends of serum ICAM-1 level in IOIP. Serum ICAM-1 level of IOIP patients with the duration of more than 12 months was （326.60 ± 38.71 ）μg·L^-1, which in the patients with the duration of six to twelve months was （264.53 ± 34.90） μg·L^-1 ,in the patients with the duration of less than six months（ 184.04 ± 53.05 ） μg·L^-1, there were statistical differences （ all P 〈 0.05 ）. The serum level of ICAM-1 in IOIP in bilateral eyes was （293.30 ±69.34）μg·L^-1 ,which in unilateral eye was （240. 17 ± 74.56 ）μg·L^-1, there was statistical difference （P 〈 0. 05 ）. Conclusion Serttm ICAM-1 level of IOIP can reflect the severity and course of disease ,testing serum ICAM-1 level of IOIP are of value not only to help make the diagnosis,but also to assess the severity, course and prognosis of the disease.
