中药结合高强度聚焦超声治疗腹膜后淋巴转移癌的研究

摘　　要：目的研究中药膈下逐瘀汤结合高强度聚焦超声(HIFU)治疗腹膜后淋巴结转移癌的临床效果。方法根据随机、对照及均衡原则,将患者分为2组各30例。试验组予HIFU结合膈下逐瘀汤治疗,膈下逐瘀汤于HIFU治疗前7 d开始服,连续服用30 d。对照组仅行HIFU治疗。对比分析2组治疗后肿瘤大小、患者疼痛感和阻塞感的改变以及KPS评分等。结果试验组治疗41个肿大淋巴结,超声复查有16个肿大淋巴结略有缩小,占39%;疼痛感减轻或消失和阻塞感减轻或消失方面,有效率分别为82%和80%;KPS评分(75.2±14.0)分。对照组治疗39个肿大淋巴结,超声复查有14个肿大淋巴结略有缩小,占28%;疼痛感减轻或消失和阻塞感减轻或消失方面有效率分别为71%和71%;KPS评分(73.2±15.3)分。结论 HIFU能够为腹膜后淋巴结转移癌的治疗提供帮助;而中药膈下逐瘀汤与其协同治疗在降阻、减痛、提高生活质量方面能有更好的治疗效果。Objective It is to observe the clinical effect of Gexia Zhuyu Tang Combined with high intensity focused ultra- sound （HIFU） in the treatment of metastatic carcinoma of retroperitoneal lymph nodes. Methods According to the random, contrast and balance principle, the patients were divided into experimental group （Gexia Zhuyu Tang Combined with HIFU） and control group （ HIFU treatment） , 30 cases in each group. The patients were treated by HIFU combined with Gexia Zhuyu Tang in the experimental group ; Gexia Zhuyu Tang was taken from 7 days before ttlFU treatment for 30 consecutive days. The patients were treated by HIFU only in the control group. The tumor size, the changes of pain and obstruction, and KPS score between the two groups after treatment were analyzed. Results Among 41 enlarged lymph nodes were treated in the experimental group, there were 16 lymph nodes slightly reduced, about 39% ; the pain relieved or disappeared and obstruction was reduce or disappear, the effective rates were 82% and 80% ; KPS score was 75.2 ± 14.0 Among 39 enlarged lymph nodes were trea- ted in the control group, there were 14 lymph nodes slightly reduced, about 28% ; pain relieved or disappeared and obstruc- tion was reduce or disappear, the effective rates were 71% and 71% ; KPS score was 73.2 ± 15.3. Conclusion HIFU can provide a helpful treatment for retroperitoneal lymph node metastasis, and combined with Gexia Zhuyu Tang has a better thera- peutic effect such as reducing feeling of obstruction and pain, improving the quality of life.
