摘 要:目的探讨全数字化钨铑靶乳腺机三维立体定位系统在乳腺病变穿刺活检术中的应用价值。方法对102例乳腺摄影发现病变的患者均行乳腺X线下三维立体定位穿刺,抽取部分组织进行活检,结合病理结果进行分析。结果 102例三维立体定位穿刺中,一次性准确定位穿刺97例,成功率为95%,3例定位穿刺多次,另有1例因乳房厚度小于射程,穿刺点无法准确选取。病理结果乳腺癌63例,占61.8%,乳腺良性病变39例,占38.2%。结论全数字化钨铑靶乳腺机三维立体定位穿刺术,具有操作简便,安全可靠,定位精确,成功率高等优点,能较早地明确乳腺病变的定性诊断,尤其对临床未触及肿块、超声阴性的早期乳腺癌具有重要的临床价值。Objective To explore the clinical application of digital mammography tungsten rhodium target three- dimensional positioning system in the puncture biopsy of breast lesions. Methods A total of 102 patients with breast lesions on mammography were enrolled in this study. Three- dimensional stereotactic localization and puncture of the breast lesions were performed in all the patients. Part of lesion was extracted and sent for pathological examination. The results were analyzed. Results Of the 102 stereotactic punctures,accurate localization puncture with single manipulation was obtained in 97 with a success rate of 95%, with multiple manipulations in 3, and in the remaining one the puncture point could not be accurately selected as the breast thickness was smaller than the exposure range. Pathological examination showed that breast cancer was confirmed in 63 cases(61.8%), and benign breast lesions in 39 cases(38.2%). Conclusion For puncture biopsy of breast lesions, digital tungsten rhodium target mammography three- dimensional positioning system has many advantages. It is easy to operate, safe and reliable with accurate localization and high success rate. This technique can precisely make early diagnosis of breast lesions, especially for clinically-impalpable breast mass and ultrasound- negative breast cancer.
