摘  要:目的:探讨单操作孔胸腔镜手术切除纵隔肿瘤的可行性。方法2013年3~8月采用单操作孔胸腔镜手术,根据术前肿瘤位置,先于腋中线第6~8肋间做长约1.5 cm切口作为观察孔,取腋前线第4或5肋间3~4 cm切口为操作孔,行纵隔肿瘤切除28例。结果28例均顺利完成手术,无中转开胸。平均手术时间56.8 min(42~107 min),平均术中出血量85 ml(40~150 ml),无手术死亡。术后带管时间2~5 d,平均3.7 d10术后住院时间5~8 d,平均6.7 d10无严重术后并发症。28例术后随访6~12个月,无局部复发。结论对于部分纵隔肿瘤采取单操作孔胸腔镜切除是安全可行的。Objective To explore the possibility and feasibility of mediastinum tumor resection by single manipulation port thoracoscopic surgery. Methods A total of 28 patients with mediastinum tumor, who were admitted to our department from March 2013 to August 2013, was analyzed.All the patients underwent single manipulation port thoracoscopic resection.Firstly, an incision 1.5 cm in length was made at 6-8 intercostal space on the midaxillary line as the observation hole.Then a manipulation port 3-4 cm in length was made at 4-5 intercostal space on the anterior axillary line, through which the operation was conducted. Results The operation was successful in all the 28 patients without conversion to open chest surgery.The average time was 56.8 min (42-107 min).The average blood loss during operation was 85 ml (40-150 ml).All the patients survived without serious complications after operation.The chest drainage tube was removed 2-5 days after operation, with an average time of 3.7 days.All the patients were discharged from hospital 5-8 days after operation, with an average time of 6.7 days.Postoperative follow-up for 6-12 months in the 28 cases found no local recurrence. Conclusion Mediastinum tumor resection by single manipulation port thoracoscopic surgery is safe and feasible.
