茯苓甘草汤治疗肺动脉高压气虚血瘀证27例  

27 cases with Fuling-Gancao Decoction in Treating Pulmonary Hypertension due to Qi-Deficiency and Blood-Stasis

摘　　要：目的:观察茯苓甘草汤治疗肺动脉高压气虚血瘀证的临床疗效。方法:将54例肺动脉高压气虚血瘀证患者随机分为对照组和试验组各27例,两组患者均给予常规治疗,对照组在常规治疗基础上口服硝苯地平控释片,试验组予茯苓甘草汤治疗,3个月后比较两组患者中医证候积分、临床疗效,并记录两组患者治疗前后肺动脉收缩压、肺功能和血液流变学指标变化。结果:治疗后试验组积分显著低于对照组(P<0.05);试验组有效率为92.6%,对照组有效率为81.5%,试验组有效率显著高于对照组(χ2=5.465,P=0.019)。治疗后两组患者肺动脉收缩压明显下降,肺功能明显改善,6 min步行试验距离显著延长(P<0.05),其中试验组各指标显著优于对照组(P<0.05);治疗后试验组血液流变学各指标均显著降低,与治疗前相比差异具有统计学意义(P<0.05),且试验组各指标又显著低于对照组(P<0.05)。结论:茯苓甘草汤能够提高肺动脉高压气虚血瘀证患者临床疗效,改善患者肺功能和肺部血液循环,疗效显著。Objective： To observe the clinical effect of Fuling-Gancao Decoction（ 茯苓甘草汤） in treating pulmonary hypertension due to qi-deficiency and blood stasis. Methods： 54 patients with pulmonary hypertension were randomly divided into the control group and the experiment group,with 27 cases in each group. The patients in both groups were given routine treatment. The ones in the control group were added adalat controlled-release tablets for oral administration; The ones in the experiment group were given Fuling-Gancao Decoction for oral administration; 3 months later,TCM syndrome score and clinical effect of patients in both groups were compared; Their pulmonary systolic blood pressure,pulmonary function and blood rheology index changes before and after the treatment were recorded. Results： After the treatment,the accumulated score of the experiment group was clearly lower than that of the control group（ P〈0. 05）; The effective rate of the experiment group was 92. 6%,while that of the control group was 81. 5%,and the experiment group was greatly higher than the control group（ χ^2= 5. 465,P = 0. 019）. After the treatment,pulmonary systolic blood pressure of the patients in both groups clearly decreased,pulmonary function greatly improved,6MWT experiment distance clearly prolonged（ P〈0. 05）,and each index of the experiment group was greatly better than those of the control group（ P〈0. 05）; After the treatment,each index of blood rheology of the experiment group was greatly lower than those before the treatment; The difference before and after the treatment had statistical meaning（ P〈0. 05）,and each index of the experiment group was clearly lower than those of the control group（ P〈0. 05）. Conclusion： Fuling-Gancao Decoction can increase the clinical effect of patients with pulmonary hypertension due to qi-deficiency and blood-stasis syndrome,improve their pulmonary function and blood circulation,which has very good effect.
