老年消化性溃疡120例胃镜检查特点及临床分析

摘　　要：目的分析老年人消化性溃疡的临床特征。方法回顾120例老年PU患者的临床资料,总结其临床表现、胃镜检查特点、Hp感染率;对幽门螺杆菌阴性患者给予奥美拉唑单独口服,对Hp阳性患者给予传统三联疗法进行治疗,观察疗效。结果 120例消化性溃疡患者中,胃溃疡66例(55%),十二指肠溃疡42例(35%),复合溃疡12例(10%);Hp阳性98例(81.7%);经奥美拉唑或传统三联疗法治疗4周后,总有效率93.3%,Hp根除率为90.8%。结论老年PU患者临床症状不典型,易漏诊;以胃溃疡发病率高,胃体中上部溃疡和巨大溃疡常见,且伴发病多、并发症发病率高。在临床上应高度重视,积极治疗。Objective Analysis the elderly peptic ulcer clinical characteristics. Methods Clinical data of 120 elderly patients with PU, summarize the clinical manifestation, endoscopic features, Hp infection rate; on Helicobacter pylori negative patients treated with omeprazole single oral, given the traditional triple therapy for Hp positive patients, curative effect observation. Results 120 cases of peptic ulcer, gastric ulcer in 66 cases （55%）, duodenal ulcer in 42 cases （35%）, 12 cases of compound ulcer （10%）; Hp was positive in 98 patients （81.7%）; 4 weeks after omeprazole or traditional triple therapy, total the efficiency of 93.3%, Hp eradication rate was 90.8%. Conclusion The clinical symptoms of elderly patients with PU is not typical, easily missed diagnosis; with high incidence of gastric ulcer, gastric ulcer and ulcer of upper body in great common, and the accompanying disease, the incidence of complications is high. In clinical practice should be paid more attention, active treatment.
