摘 要:目的探讨5-羟色胺转运体启动子区(5-HTTLPR)基因多态性与抑郁症的关系以及基因型、应激事件对抑郁症状的影响。方法运用聚合酶链式反应和限制性片段长度多态性(PCR-RFLP)测定100例抑郁症患者(研究组)和100例健康对照者(对照组)的5-HTTLPR基因多态性的分布频率。应用汉密尔顿抑郁量表(HAMD)评定抑郁症患者的严重程度,应用生活事件量表(LES)评定抑郁症患者的应激事件。结果两组5-HTTLPR基因型频率和等位基因频率比较差异均无统计学意义(P>0.05);研究组SS基因型的HAMD总分大于LL、LS基因型患者(P<0.05);SS基因型患者焦虑/躯体化、核心因子、睡眠因子分高于SL及LL基因型患者(P<0.05);5-HTTLPR基因多态性与应激事件的交互作用对抑郁症的严重程度存在影响(P=0.002)。结论 5-HTTLPR基因多态性可能与抑郁症的发病无关,但与应激事件的交互作用影响抑郁症严重程度,其基因型与抑郁症患者的临床症状有关。Objective To explore the relationship between polymorphism of 5-HT transporter gene - linked promoter region (5-HTTLPR) and depression, to explore the effect of genotype and stress events on depression. Methods The distribution of 5- HTYLPR polymorphism was detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) inl00 patients with depression and 100 healthy controls. Hamilton depression rating scale (HAMD) was used to evaluate the severity of depression and the life events scale (LES) was used to assess the stress events. Results There were no significant differences in 5-HTTLPR genotype frequency and allele frequency between depression group and control group ( P 〉 0.05 ). Score of HAMD in depressive patients with SS genotype was significantly higher than those with LL and LS genotype ( P 〈 0.05 ). Factor scores of anxiety/somatization, core factor and sleep factor in depressive patients with SS genotype were significantly higher than those with SL and LL genotype ( P 〈 0.05 ). The interaction between 5-HTTLPR polymorphism and life events affected the severity of depression significantly (P = 0. 002 ). Conclusion The polymorphism of 5-HTYLPR may not be associated with the pathogenesis of depression, but the interaction of 5-HTTLPR gene polymorphism with stress events may influence the severity of depression. And the genotypes of 5-HTYLPR are associated with different depressive symptoms.
