摘 要:目的检测慢性牙周炎患者唾液β-葡萄糖醛酸苷酶(β-glucuronidase,β-G)水平,并与治疗后及健康人群对照,以期明确β-G与慢性牙周炎的关系,为牙周炎诊断及疗效监测提供有效参考。方法选择18名慢性牙周炎患者,采集牙周基础治疗前及治疗后全唾液并记录牙周临床指数;检测全唾液中β-G(比色法)水平,并分析与牙周临床参数之间的相关性。结果 (1)基础治疗后,牙周临床参数改善明显(P<0.05);(2)治疗前实验组唾液β-G活性水平(2.845±0.72,10-3U·m L-1)与健康对照组(2.24±0.31,10-3U·m L-1)差异有显著性(P<0.05),与平均牙周袋深度(r=0.485)、深袋数(r=0.540)、深袋构成比(r=0.598)之间明显相关(P<0.05);治疗后β-G酶活性水平(2.18±0.71,10-3U·m L-1)明显下调(P<0.05),与对照组差异无显著性(P>0.05)。结论唾液β-G活性水平能在一定程度上反映当前牙周炎状态及控制程度,该指标可为牙周炎诊断和疗效监测提供参考。Objective The aims of study were to investigate the changes of the salivary levels ofβ-G in patients with chronic periodonti-tis before and after initial periodontal therapy,and evaluate the relationship between the content of the enzyme and periodontal clinical parameters.Methods Eighteen patients with moderate to severe chronic periodontitis were selected.They first underwent supragingival scaling and oral health instruction ( OHI) .A week later,non-stimulated whole saliva was collected and periodontal clinical parameters were examined and the patients were treated with subgingival scaling and root planning.The salivaryβ-G was examined by flourometric assay.A month later,the salivary levels ofβ-G were reevaluated.t test was used to analyze the differences of salivary enzymes levels and periodontal parameters before and after initial periodontal therapy;pearson correlation was used to analyze the correlation between the levels of enzymes and periodontal parameters.Results (1) The changes in the clinical periodontal variables were significant following initial periodontal therapy;(2) At baseline,the salivaryβ-G activity of chronic periodontitis patients was statistically higher than that of controls (P〈0.05),and correlated significantly with mean probing depth (MPD) and the number of sites with probing depth≥6 mm. Following initial periodontal therapy,the salivaryβ-G activity decreased significantly.Conclusion The salivaryβ-G activity could be a reflection of current status of chronic periodontitis.The detection of the enzyme may therefore aid clinician in the diagnosis and thera-peutic evaluation of the disease.
