济泰片与美沙酮治疗社区戒毒人员焦虑症状的疗效观察

摘　　要：目的:比较济泰片与美沙酮治疗社区戒毒人员焦虑状况的疗效。方法:对上海7个区65个街道且符合入组标准的386例社区戒毒人员进行为期2年的开放性队列对照研究。研究对象分为济泰片组(206例)和美沙酮组(180例)。济泰片组给予济泰片口服治疗1年,停药后继续观察1年,美沙酮组给予美沙酮维持治疗2年,且在第1年内对两组同时合并实施团体心理咨询和社会支持干预。采用焦虑自评量表(SAS)分别在7个随访时点对其焦虑状态进行测评。结果:在改善焦虑症状方面,济泰片组SAS标准评分的中位数从41分下降到36分,美沙酮组从44分下降到41分;经混合效应模型检验及多重比较,两组焦虑标准评分在时间和组别两个效应上的差异均有统计学意义(P<0.05),即从基线到104周末总体上呈现下降趋势且济泰片组较美沙酮组下降略多。结论:在社区戒毒康复中,济泰片与美沙酮治疗合并团体心理咨询和社会支持干预均能改善社区戒毒人员的焦虑症状,且济泰片的效果略优于美沙酮。Objective： To compare the curative effect of treating anxiety symptom of community rehabilitation personnel with Jitai tablets and methadone. Methods： A two-year cohort study was conducted, 386 eligible subjects from 7 districts and 65 communities in Shanghai were included in the cohort, and study objects were divided into Jitai Tablets group（206 cases） and methadone group（180 cases）. The Jitai Tablets group was treated with Jitai tablets for one year, after the treatment they were observed for another year. The methadone group was treated with methadone for two years. Both groups were intervened with psychological intervention and social support in the first year during their treatment. Their degree of anxiety was measured by SelfRating Anxiety Scale（SAS） at seven different follow-up time points. Results： The median of SAS scores of Jitai Tablets group has descended from 41 to 36, while the median of SAS scores of methadone group has descended from 44 to 41. Mixed model test and multiple comparisons indicated that there was significant difference in group effect and time effect（P〈0.05）. It means that the SAS scores of both groups showed a downward trend from baseline to 104 weekends, and the SAS scores of Jitai tablets group fallen slightly more than that of methadone group. Conclusion： Methadone, Jitai tablets combined with psychological intervention and social support both could improve the anxiety symptom of community rehabilitation personnel, and the effect of Jitai tablets was slightly better than that of methadone.
