无覆盖带蒂颊脂垫瓣在儿童腭裂修复手术松弛切口中的临床运用

摘　　要：目的探讨无覆盖带蒂颊脂垫瓣在儿童腭裂修复手术松弛切口中的临床应用价值,以提高患者临床治疗效果。方法选取该院2011年5月—2014年7月收治的60例行腭裂修复手术的患儿为研究对象,按照收治时间分为对照组与研究组各30例,对照组采用碘仿纱条疗法;研究组采用无覆盖带蒂颊脂垫瓣进行治疗,比对两组患者临床治疗效果及指标变化情况。结果研究组通过采用无覆盖带蒂颊脂垫瓣进行治疗,总有效率93.3%、并发症发生率20%,与对照组73.3%、40%相比较,差异具有统计学意义(P<0.05);术后白蛋白(185.8±20.2)mg/L、持续低热时间(2.5±1.0)d,与对照组(202.7±19.5)mg/L、(5.5±1.5)d相比较,差异有统计学意义(P<0.05)。结论无覆盖带蒂颊脂垫瓣具有疗效确切、并发症发生几率低等特点,能够显著改善腭裂患儿症状,具有较高的临床应用价值,值得在今后临床工作中推广使用。Objective To investigate the clinical application value of uncovered pedicled buccal fat pad flap in pediatric cleft palate repair surgery relaxing incision so as to improve the clinical treatment effect on the patients. Methods 60 children with cleft palate repair surgery admitted in our hospital from May 2011 to July 2014 were selected as the subjects and divided into the control group and the study group with 30 children in each according to the admission time. The control group was treated with iodoform gauze, and the study group was treated with uncovered pedicled buccal fat pad flap. And the clinical treatment effect and changes of indicators were compared between the two groups. Results The total effective rate and incidence of complications in the study group treated by uncovered pedicled buccal fat pad flap was 93.3%, 20%, respectively, and that of the control group was73.3%, 40%, respectively, the difference between the groups was statistically significant（P〈0.05）; the postoperative albumin and duration of low-grade fever was（185.8±20.2） mg/L,（2.5±1.0） d, respectively, and that of the control group was（202.7±19.5） mg/L,（5.5±1.5） d, respectively, the difference between the two groups was statistically significant（P〈0.05）. Conclusion Uncovered pedicled buccal fat pad flap has the characteristics of definite effect and low incidence of complications, which can obviously improve the symptoms of children with cleft palate with high clinical value, so it is worthy of clinical application and promotion.
