摘 要:目的研究帕金森病(PD)患者红细胞α-突触核蛋白寡聚体以及其与总蛋白的比值能否作为帕金森病的外周生物学标记物。方法选择PD患者77例(PD组),多系统萎缩(MSA)患者22例(MSA组),健康体检者89例(对照组)。红细胞内总蛋白通过蛋白定量的方法检测,α-突触核蛋白寡聚体通过ELISA检测。结果与对照组比较,PD组和MSA组红细胞α-突触核蛋白寡聚体含量[(3887±2160)μg/L,(3844±1887)μg/L vs(2718±887)μg/L],及红细胞α-突触核蛋白寡聚体/总蛋白[(28.5±18.1)ng/mg,(22.3±13.8)ng/mg vs(14.3±7.4)ng/mg)]明显升高(P<0.01)。在红细胞α-突触核蛋白寡聚体作为PD标记物的诊断中,ROC曲线下面积为0.68(P=0.037),最佳临界值为3801.8g/L,敏感性为42%,特异性为36%。而其与红细胞总蛋白的比值作为诊断指标,ROC曲线下面积为0.75(P=0.031),最佳临界值为14.90ng/mg,敏感性为76%,特异性为52%。结论红细胞α-突触核蛋白寡聚体以及红细胞α-突触核蛋白寡聚体/总蛋白的比值可作为鉴别突触核蛋白病与非突触核蛋白病的指标,后者有望成为诊断PD的外周生物学指标。Objective To study whether α-synuclein oligomer and the ratio betweenα-synuclein oli gomer and total protein in RBC can be used as biomarkers for PD. Methods Seventy seven PD patients served as a PD group, 22 multiple system atrophy(MSA)patients served as a MSA group,and 40 subjects undergoing physical examination served as a control group in this study. The total protein in RBC was detected by BCA and the α-synuclein oligomer was tested by EI.ISA. Results The serum α-synuclein oligomer level and the ratio between α-synuclein oligomer and total protein in RBC were significantly higher in PD group and MSA group than in control group (P〈0.01). The area under ROC curve was 0.68 when α-synuclein oligomer was used as a biomarker for PD (P=0. 037) with a cut-off value of 3801.8 mg/ml,a sensitivity of 42% and a specificity of 36 %. The area under ROC curve was 0.75 when the ratio between α-synuelein oligomer and total protein in RBC was used as a biomarker for PD (P=0. 031)with a cut-off value of 14.90 ng/mg,a sensitivity of 76% and a specificity of 52%. Conclusion The serum α-synuclein oligomer level and the ratio between α-synuclein oligomer and total protein in RBC can be used as biomarkers for differential diagnosis of synucleinopathy from non-synucleinopathy. The ratio be tween α-synuclein oligomer and total protein in RBC may become a peripheral biomarker for PD.
