^(32)磷-胶体介入治疗甲状腺瘤的临床观察

摘　　要：目的观察超声引导下的核素32磷-胶体(32P-胶体)瘤体内注射介入治疗甲状腺瘤的效果。方法以分别接受32P-胶体瘤体内注射介入治疗(32P-胶体组,n=30)和超声引导下经皮穿刺瘤内无水乙醇注射治疗(对照组,n=30)的甲状腺瘤患者作为研究对象。分别于治疗后2、4、6、8个月时随访疗效及不良反应的发生情况。结果治疗后定期随访发现,32P-胶体组患者接受治疗6个月和8个月后的疗效与瘤体大小有关(P=0.042,P=0.019);治疗后8个月,32P-胶体组疗效显著优于对照组((P=0.011)。32P-胶体组的不良反应主要为肿瘤部位轻度胀痛(33.00%)、局部皮肤发红(16.67%)及放射性皮炎(6.70%)。结论超声引导下32P-胶体瘤体内注射介入治疗甲状腺腺瘤,操作简便、效果显著且不良反应少;其疗效与瘤体大小有关。Objective To observe the effects of interventional therapy on the thyroid tumor by intratumoral injection of ^32Phosphorus-radiocolloid （^32 P-colloidal） under ultrasonic guidance. Methods Patients with thyroid tumors who underwent intratumoral injection of ^32P-colloidal （^32 P-colloidal group, n =30） and pure ethanol （control group, n =30） under ultrasound guidance were selected. The therapeutic effect and adverse reactions were recorded by follow-up after being treated for 2, 4, 6, and 8 months. Results Regular follow- up showed that the therapeutic effect of ^32P-colloidal group was correlated with the tumor size after being treated for 6 and 8 months （P=0. 042 and 0. 019, respectively）. The therapeutic effect of ^32P-colloidal group was significantly better than that of the control group after being treated for 8 months （P =0. 011）. The main adverse reactions of ^32P-colloidal group were mild pain at tumors sites （33.00%）, local skin erubescence （16. 67%）, and radiodermatids （ 6. 70% ）. Conclusion The intervendonal therapy of thyroid tumor by intratumoral injection of ^32P-colloidal under ultrasonic guidance is convenient and effective with less adverse reactions. The therapeutic effect is correlated with the tumor size.
