摘 要:目的 探讨Delta样配体4(DLL4)和低氧诱导因子1α(HIF-1α)在乳癌组织的表达及其临床意义。方法 采用免疫组化方法,检测122例乳房浸润性导管癌(其中Ⅰ级33例,Ⅱ级40例,Ⅲ级49例)组织中DLL4和HIF-1α的表达,分析两者表达与乳癌临床病理参数的相关性。结果 DLL4和HIF-1α在乳房浸润性导管癌低级别组(Ⅰ级、Ⅱ级)和高级别组(Ⅲ级)中的表达阳性率分别为63.0%、91.8%和56.2%、89.8%,差异有统计学意义(χ^2=12.852、15.695,P〈0.05),但HIF-1α在Ⅰ级和Ⅱ级间的表达差异无统计学意义(P〉0.05)。DLL4在乳癌组织中的表达与组织学分级、肿瘤大小、临床分期、淋巴结转移有关(χ^2=4.802-12.852,P〈0.05),与病人年龄、雌激素受体(ER)、孕激素受体(PR)、HER2表达无相关性(P〉0.05)。HIF-1α在乳癌组织中的表达与组织学分级和淋巴结转移相关(χ2=8.961、15.695,P〈0.05),与病人年龄、肿瘤大小、ER、PR、HER2表达无关(P〉0.05)。乳癌DLL4阳性表达病人HIF-1α表达阳性率明显高于DLL4阴性表达病人,两者表达水平呈正相关(r=0.270,P〈0.01)。结论DLL4和HIF-1α可能共同调控乳癌肿瘤血管生成并影响乳癌的发展及预后,可作为预测乳癌预后的重要参考指标。Objective To explore the expression of Delta-like Ligand 4 (DLL4) and hypoxia-inducible factor 1α (HIF-1α) in breast cancer and its clinical significance. Methods Immunohistochemical staining was used to detect the expression of DLL4 and HIF let in 122 cases of infiltrating duct cancer of the breast-grade T 33 cases, grade Ⅱ ,40, and grade Ⅲ ,49. The correlation between the two expressions with clinieopathologie parameters was analyzed. Results The positive rate of DLL4 and HIF-1α in the low grade (grade Ⅰ and Ⅱ ) cancer group was 63.0% and 91.8%, respectively, and that in the high-grade cancer group was 56.2% and 89.8%, the differences being statistically significant (x^2 = 12.852,15.695 ;P〈0.05). While the difference of HIF-1α expression between the grade Ⅰ and grade Ⅱ group was not significant (P〉0.05). The expression of DLL4 in the breast cancer was associated with histological grade, tumor size, clinical stage and lymph node metastasis (x^2 =4.802-12.852 ,P〈0.05), and unrelated with patient age, ER,PR and HER2 expression (P〉0.05). The expression of HIF-la in patients with positive DLL4 was significantly higher than in those with negative DLL4 (x^2 =8.961,15.695 ;P〈0.05), the two expressions were positively correlated (r=0.270, P〈0.01). Conclusion DLL4 and HIF-1α are likely to co-regulate angiogenesis of the breast cancer and affect the development and prognosis of this malignancy, which can be used as an important reference mark for predicting its prognosis.
