广泛性焦虑障碍病人心理健康水平与血浆皮质醇关系

摘　　要：目的 了解广泛性焦虑障碍病人的心理健康水平与血浆皮质醇水平的关系。方法 应用汉密尔顿焦虑量表（HAMA）及症状自评量表（SCL-90）,对38例广泛性焦虑障碍病人、39例心理正常者（对照组）焦虑程度及心理健康水平进行评估,并使用电化学发光法检测其血浆皮质醇水平。结果 广泛性焦虑障碍病人SCL-90总分及9个因子分显著高于对照组（t=2.129-13.971,P〈0.01）。广泛性焦虑障碍病人血浆皮质醇浓度高于对照组,差异有显著性（t=23.802,P〈0.01）。血浆皮质醇浓度与SCL-90总分、躯体化、强迫状态、人际关系敏感、抑郁、焦虑、恐怖等评分均呈显著正相关（r=0.501-0.992,P〈0.01）。结论 广泛性焦虑障碍病人的血浆皮质醇水平升高,且其血浆皮质醇水平可能与心理健康水平相关。Objective To understand the correlation of mental health with plasma cortisol levels in patients with generalized anxiety disorder. Methods Hamilton Anxiety Scale （HAMA） and Symptom Checklist-90 （SCL-90） were used to assess the degree of anxiety and the level of mental health in 38 patients with generalized anxiety disorder and 39 normal controls, their plasma cortisol levels were detected using electrochemiluminescence. Results The SCL-90 total score and nine subscores in the patients were significantly higher than that in normal controls （t = 2.129-13.971, P 〈0.01）. Plasma eortisol levels in the patients were significantly higher than that in the normal controls （t = 23.802, P 〈0.01）. Plasma cortisol concentrations were markedly po sitively correlated with total scores of SCL-90, of somatization, of obsessive-compulsive, of interpersonal sensitivity, of depres sion, of anxiety and of photic anxiety （r=0.501-0.995,P〈0.01）. Conclusion Plasma cortisol concentrations in the patients with generalized anxiety disorder are elevated, which may associate with the level mental health.
