摘 要:目的探讨宫颈鳞癌(cervical squamous cell carcinomas,CSCC)组织中表皮生长因子受体(EGFR)、Rab5a、Rab21蛋白表达及其与临床特征及近期疗效的关系,评估EGFR、Rab5a、Rab21蛋白在CSCC进展、转移及近期疗效中的价值。方法收集60例CSCC组织标本,采用免疫组织化学法检测EGFR、Rab5a、Rab21蛋白表达情况。采用卡方或确切概率法、非参数Spearman等级相关、多因素Logistic回归分析法分析其与临床特征及近期疗效的相关性。结果 EGFR、Rab5a、Rab21蛋白阳性表达率分别为70.0%、73.3%、55.0%,EGFR、Rab5a、Rab21蛋白表达均与肿瘤分化程度、淋巴结转移有关(P<0.05),EGFR蛋白表达与临床分期有关(P=0.005),Rab21蛋白表达与肿瘤直径有关(P=0.044);Spearman等级相关分析显示:EGFR与Rab5a蛋白表达水平呈正相关(r=0.428,P=0.001),EGFR与Rab21蛋白表达水平呈负相关(r=-0.300,P=0.020);EGFR、Rab5a、Rab21蛋白表达均不是影响近期疗效的独立因素(P>0.05),EGFR、Rab5a、Rab21蛋白表达对客观有效率RR无明显影响(P>0.05)。结论 EGFR蛋白表达可能为CSCC高危进展的参考指标;Rab5a在EGFR介导的信号传导通路中可能发挥正性调节作用;Rab21在EGFR介导的信号传导通路中可能发挥负性调节作用。Objective To investigate the relationship between EGFR, Rab5a, Rab21 protein expression and clinical features and the short-term efficacy in cervical squamous cell carcinoma. Methods Immunohisto- chemistry method was used to detect EGFR, Rab5a, Rab21 protein expression levels in 60 cases of CSCC. Chi-Square, Spearman rank correlation, Logistic regression analysis method were used. Results The pro- tein expression rates of EGFR, Rab5a, Rab21 were 70.0%, 73.3%, 55.0%, EGFR, Rab5a, Rab21 protein expression were correlated with lymph node metastasis and tumor differentiation (P 〈0.05) ; EG- FR protein expression was correlated with clinical stage (P =0.005) ; Rab21 protein expression was corre- lated with the diameter of tumor (P =0.044) ; Spearman rank correlation analysis showed that EGFR was positively correlated with Rab5a (r = 0.428, P = 0.001) and EGFR was negatively correlated with Rab21 expression (r =--0.300, P =0.020). EGFR, Rab5a and Rab21 protein expression were not the independ- ent factors of the short-term efficacy in CSCC (P 〉0.05); EGFR, Rab5a and Rab21 protein expression had no significant effect on objective efficiency (P 〉0.05). Conclusion EGFR protein expression may be the reference index of progression of CSCC; Rab5a may play a positive role in the regulation of EGFR-me- diated signal transduction pathway while Rab21 may play a negative role.
