5种肺癌标志物在血清中的表达分布及组合检测的分类学价值

摘　　要：目的研究5种肺癌标志物在血清中表达分布及组合检测的分类学价值。方法应用免疫发光方法,检测143例肺癌患者和52例非良性疾病患者血清中的5种肺癌标志物的表达水平,采用统计学分析它们组合检测的分类学价值。结果不同病理分型肺癌患者血清细胞角蛋白19可溶性片段(CYfra21-1)、神经元特异烯醇化酶(NSE)、糖类抗原-153(CA-153)、鳞状细胞癌抗原(SCC-Ag)、胃泌素释放肽前体(pro-GRP)表达水平均高于肺良性病变组(P<0.01);NSE和pro-GRP在小细胞肺癌(SCLC)患者血清内的表达水平明显高于非小细胞肺癌(NSCLC)患者组(P<0.01);SCC-Ag在肺腺癌和鳞癌中的表达水平高于大细胞肺癌(LCLC)和小细胞肺癌(P<0.05)。结论组合检测NSE+pro-GRP可以把肺癌区分为小细胞肺癌和非小细胞肺癌两大类别;SCC-Ag+CA-153更利于从非小细胞肺癌患者中做出鳞癌的分型诊断;CYfra21-1+CA-153可以对腺癌作出鉴别诊断;SCC-Ag在T1、T2期的表达水平高于其他标志物(P<0.05)。Objective To investigate the expressing distribution of five lung cancer markers in serum and taxonomic values of their panel detection. Methods The levels of five lung cancer serum expressing markers in 143 patients with lung cancer and52 patients with benign diseases were measured by chemiluminoimmunoassay and the taxonomic values of their panel detection were analyzed by statistics. Results All of CYfra21- 1,NSE,CA- 153,SCC- Ag,pro- GRP serum expressing levels of patients with various pathologically taxonomic lung cancers were higher than those of patients with benign diseases（ P〈0. 01）;NSE and pro- GRP serum expressing levels in patients with SCLC were significantlly higher than those in patients with NSCLC（ P〈0. 01）; There were differences with statistical significances between SCC- Ag expressing levels in adenocarcinoma and squmous cell carcinoma and in SCLC and LCLC（ P〈0. 05）. Conclusion The panel detection of NSE ＋ pro- GRP could divide lung cancers into two categories： SCLC and NSCLC; The measurement of SCC- Ag ＋ CA- 153 could help taxonomically diagnose squmous cell carcinoma from NSCLCs,and CYfra21- 1 ＋ CA153 could identify adenocarcinoma; SCC- Ag expressing levels during clinically stage T1 and T2were higher than those of other makers（ P〈0. 05）.
