急性脑梗死患者动脉溶栓后神经功能改善程度与血小板活化状态

摘　　要：目的观察急性脑梗死患者动脉溶栓对神经功能的改善程度及血小板活化状态的变化,探讨二者间的临床联系及机制。方法选取急性脑梗死患者36例,行重组组织型纤溶酶原激活剂(recombinant tissue plasminogen activator,rt-PA)选择性动脉内溶栓治疗。每位患者在溶栓治疗前、治疗后1 d、7 d及30 d分别检测静脉血血小板活化状态标志物CD62P、CD63及血栓素B2(thromboxare B2,TXB2)水平;分别进行神经功能评估。结果溶栓后,患者CD62P、CD63及TXB2下降(P<0.05);显效组患者CD62P、CD63及TXB2下降程度高于无效组(P<0.05)。结论急性脑梗死患者动脉溶栓后显效者血小板活化降低程度高于无效者,观察血小板活化状态变化有助于对患者治疗效果及预后的判断。Objective To observe the amelioration of nerve functions and alteration of platelet activation status in the patients with acute cerebral infarction and investigate their relation.Methods A total of 36 cases with acute cerebral infarction were treated through rt-PA intrwarterial thrombolysis after cerebral angiography.Before treatment and 1 d,7 d, 30 d after treatment,CD62P,CD63 and TXB2 were detected, nerve functional assessment were performed with National Institute of Health stroke scale （NIHSS） among all the patients.Results After treatment,CD62P.CI）63 and TXB2 de- scended （ P 〈0.05）.The descend in patients with excellence effect was higher than that in patients without effect （P〈 0.05）.Conclusion Among the patients with intra-arterial thrombolytic therapy, the descend of platelet activation is higher in those with excellence effect than in those without effect.Observing the descend of platelet activation may help to predict the treatment effect and prognosis of the patiens.
