摘 要:目的探讨PI3K、Akt、mTOR、p70S6K在宫颈癌变过程中的表达。结合宫颈病变的临床病理学特征,对4个基因蛋白阳性表达的相关性进行临床研究。方法选取20例宫颈糜烂伴鳞状上皮化生、60例宫颈上皮内瘤变(CINⅠ、CINⅡ、CINⅢ各20例)以及48例宫颈鳞状细胞癌患者,应用免疫组织化学SP法检测各组宫颈组织中PI3K、Akt、mTOR、p70S6K的表达,选取10例正常宫颈组织作为对照组。结果 1在宫颈癌变动态过程中,从宫颈糜烂伴鳞化、宫颈上皮内瘤变到宫颈鳞状细胞癌,PI3K、Akt、m TOR、p70S6K癌基因阳性表达率逐渐增高,差异有统计学意义(P<0.05)。2PI3K、Akt、mTOR与p70S6K在宫颈癌中的阳性表达与临床分期、肿瘤分化程度、淋巴结转移有关(P<0.05),淋巴结转移组阳性表达率明显高于无淋巴结转移组(P<0.01),与年龄、病灶大小无关(P>0.05)。3PI3K、Akt、mTOR和p70S6K与宫颈上皮内瘤变和在宫颈癌中的阳性表达呈正相关。结论 PI3K、Akt、mTOR、p70S6K在宫颈癌变过程中存在着联合表达,并协同参与了宫颈癌的发生和进展,可以作为早期诊断、预后判断的新的生物学标志物,并为基因的协同治疗提供新靶点。【Objective】 To evaluate the expressions of PI3 K, Akt, m TOR and p70S6 K in the successive steps of progression in cervical neoplasia and to determine their correlation with clinicopathologic features in squamous cell carcinoma of the uterine cervix. 【Methods】 Immunohistochemical staining with anti-PI3 K, antiAkt, anti-m TOR and anti-p70S6 K antibodies was performed in a total of 138 patients with 10 normal cervical epithelium, 20 cervical erosion metaplasia, 60 cervical intraepithelial neoplasia(20 CIN Ⅰ, 20 CIN Ⅱ, 20 CIN Ⅲ), and 48 cervical squamous cell carcinomas(20 patients of Stage Ⅰ, Ⅱ; 28 patients of stage Ⅲ, Ⅳ).【Results】 1PI3 K, Akt, m TOR and p70S6 K expressions progressively increased along the continuum from normal epithelium to squamous cell carcinoma(P〈0.05). 2Positive PI3 K, Akt, m TOR and p70S6 K immunostaining was associated with clinical stage(P〈0.05), the differentiation degree(P〈0.05) and pelvic lymph node metastasis(P〉0.01), but not related to the age or tumor size(P〈0.05). 3The expression of PI3 K, Akt and m TOR positively correlated to p70S6 K expression in cervical intraepithelial neoplasia and cervical squamous cell carcinoma(P〈0.01). 【Conclusion】 Reduced expressions of PI3 K, Akt, m TOR and p70S6 K proteins may be valuable biomarkers that are predictive of the progression and prognosis in patients with cervical squamous cell carcinoma. These useful biomarkers may have potential as promising therapeutic targets.
