摘　　要：目的:评价中医药治疗非痴呆血管性认知功能障碍(VCIND)的临床疗效和安全性。方法:选用国内权威数据库中国知识资源总库(CNKI)、维普中文期刊数据库(VIP)、万方学术期刊全文数据库、中国生物医学文献服务系统(CBM)等作为治疗源。选择在针对血管危险因素的治疗基础上运用中药(治疗组)与西药(对照组)治疗VCIND的临床随机对照试验(RCT)文献,经Jadad量表评分2分以上的文献纳入研究,进行Meta分析。结果:经严格筛选,最终纳入文献19篇,涉及1517例患者,但其方法学质量均较低。与对照组比较,治疗组治VCIND在总有效率和改善简单智能精神状态检查量表(MMSE)评分、蒙特利尔认知评估量表(MOCA)评分、行为能力量表(Blessed)评分方面有统计学意义;纳入文献3例出现不良反应。结论:中医药治疗VCIND有效性和安全性获益具有潜在的优势,但还需要设计良好的随机对照及多中心临床试验做进一步证实,不良反应方面亦有待于今后的临床研究提供依据。Objective： To evaluate the clinical efficacy and safety of Chinese medicine treatment of vascular cognitive impairment without dementia（ VCIND）. Methods： The domestic authoritative databases,such as China knowledge infrastructure（ CNKI）,VIP Chinese journal database（ VIP）,Wanfang academic periodical full text database,China biomedical literature service system（ CBM） and so on were as the sources of information. The use of traditional Chinese medicine in the treatment（ treatment group） and Western medicine（ control group） randomized clinical trials（ RCTs） with the Jadad score more than 2 points literatures were included in the study and the Meta- analysis was carried on. Results： After the strict screening,19 researches were included finally,involving 1517 patients. But their methodological quality was low. Compared with the control group,the treatment group＇s mini- mental state examination（ MMSE） score,Montreal cognitive assessment scale（ MOCA） score and behavior scale（ Blessed） score were statistically significant. In the analysis,only 3 cases appeared adverse reactions. Conclusion： The efficacy and safety of Chinese medicine treatment of VCIND has potential advantages. But it also needs to design a good randomized and multi- center clinical trial for further confirmation. Adverse reaction has also to be provided on basis for future clinical research.
