高血压患者血清同型半胱氨酸水平及亚甲基四氢叶酸还原酶基因C677T多态性观察研究

摘　　要：目的探讨亚甲基四氢叶酸还原酶（MTHFR）基因C677T多态性与高血压的关系。方法从我院心内科门诊及病房收集的230例原发性高血压患者；对照组为我院健康体检者200例。收集高血压患者及健康体检者的血清同型半胱氨酸（HCY）水平、MTHFRC677T位点的基因型及其他相关指标。结果①高血压组与对照组血清HCY比较差异有统计学意义（P〈0．05）。②高血压组与对照组比较，前者MTHFRC677T位点的基因型中杂合型（CT型）及突变型（TT型）比例较后者有所升高（P〈0．05）。③其中2、3级高血压亚组患者CT型及TT型比例升高较1级高血压亚组更明显（P〈0．05）。结论高血压患者血清同型半胱氨酸水平较正常人升高，其MTHFRC677T的基因型中杂合型及突变型比例较正常人亦有所升高，其中，中重度高血压者升高更加明显，提示血清同型半胱氨酸水平及MTHFRC677T位点的基因型对高血压具有一定的作用。Objective To explore the relationship between serum homocysteine （HCY） level, methylenetetrahydrofolate reductase （MTHFR） gene C677T polymorphism and primary hypertension. Methods We collected 230 primary hypertension patients from the Cardiovascular Department of Shenzhen No. 2 People Is Hospital outpatient and ward who were 20 - 78 years old; and 200 healthy subjects as control group from the Medical Department of Shenzhen No. 2 People＇s Hospital. The serum HCY level, MTHFR genotype C677T polymorphismand other related indicators were determined in hypertensive patients and healthy subjects. Results Compared with the control group, the serum HCY level of the hypertension group was higher than that of the healthy subjects, the difference was statistically significant （P 〈 0.05 ）. Compared with the control group, the ratio of MTHFR genotype C677T locus in heterozygous type and mutant in the hypertension group was higher than the latter, the difference was statistically significant （ P 〈 O. 05 ）. In grade 2 and grade 3 hypertension patient subgroups heterozygous type and mutant ratio was higher than that of grade 1 hypertension group, the difference was statistically significant （P 〈 0.05 ）. Conclusion Serum HCY levels are elevated in patients with hypertension, and the MTHFR genotype in C677T heterozygous and mutant ratio are also increased significantly, especially in severe hypertension group. Serum HCY level and the gene type of MTHFR C677T site have certain effect on hypertension.
