中医综合治疗老年晚期结直肠癌临床观察

摘　　要：目的观察辨证施治联合口服中成药、中药注射剂、针灸中医综合治疗老年晚期结直肠癌的临床疗效。方法将60例确诊为晚期结直肠癌患者随机分为治疗组(接受中医综合治疗)和对照组(接受FOLF0X方案化疗),观察2组治疗效果和不良反应发生情况。结果 2组均无完全缓解(CR)者,治疗组有效率(RR)为43.7%,对照组为50.0%,差异无统计学意义(P>0.05);治疗组疾病控制率(DCR)为87.5%,对照组为89.2%,差异无统计学意义(P>0.05)。中位疾病进展时间(TTP)治疗组为8月,对照组为7月;中位生存期(OS)治疗组为16月,对照组为14月,2组比较差异均无统计学意义(P>0.05)。药物不良反应:治疗组中、重度肝损害发生率低于对照组,骨髓抑制、胃肠道反应、心脏及神经系统不良反应治疗组明显少于对照组(P<0.05或P<0.01)。治疗组生存质量的改善情况优于对照组。结论中医综合治疗方案在治疗老年晚期结直肠癌方面较化疗具有不良反应少,生存质量明显提高的优势。Objective To observe the clinical efficacy of combination treatments for elderly patients with advanced colorectal cancer,including oral Chinese patent medicine,traditional Chinese medicine（TCM） injection,acupuncture and so on.Methods Sixty elderly patients with advanced colorectal cancer were randomly divided into research group（receiving comprehensive treatment of TCM） and control group（receiving FOLFOX）.Results There were no patients with complete remission（CR） in two groups after treatment.The effective rate was 43.7%in research group,and 50.0%in control group.The disease control rates（DCR） was 87.5%in research group,and 89.2%in control group.There was no significant difference between two groups（P〉0.05）.The median time to progressive disease（MTTP） was 8 months in research group,and 7 months in control group（P〉0.05）.The median survival time was 16 months in test group,and 14 months in control group（P〉0.05）.The incidence of severe liver damage was significantly less,and some toxic reaction such as myelosuppression,gastrointestinal reaction,cardiotoxicity,neurotoxicity were slighter in research group than those in the control group（P〈0.05 or P〈0.01）.Conclusions Compared with chemotherapy,TCM comprehensive therapeutic has the similar effect,less toxicity,and could significantly improve the quality of life in the elderly patients with advanced colorectal cancer.
