立体定向血肿穿刺抽吸术联合醒脑静注射液治疗高血压脑出血的临床研究

摘　　要：目的探讨立体定向血肿穿刺抽吸术联合醒脑静注射液治疗高血压脑出血的效果。方法 42例高血压脑出血患者采取立体定向血肿穿刺抽吸术联合醒脑静治疗为病例组,同期接受开颅血肿清除术治疗的高血压脑出血患者42例作为对照组。结果病例组的治愈率显著高于对照组(P<0.01),死亡率也明显低于对照组(P<0.01)。两组总体疗效相比,病例组明显优于对照组(u=2.7354,P<0.01)。病例组肺部感染、消化道出血及二次出血等并发症例数均低于对照组(P<0.05);而病例组总的并发症发生率也明显低于对照组(P<0.01)。病例组患者生活自理例数高于对照组(P<0.05);而病例组患者不能自理的例数则明显低于对照组(P<0.01)。病例组治疗后GCS评分显著高于对照组同期GCS评分(P<0.01),也明显高于该组治疗前(P<0.01)。结论立体定向血肿穿刺抽吸术联合醒脑静治疗高血压脑出血,可以提高高血压脑出血治愈率,降低死亡率,减少并发症,提高安全性,减轻患者昏迷程度,并减少远期致残率,较传统开颅术优势明显。[Objective] To investigate the efficacy Xingnaojing Injection in the treatment of hypertensive of stereotactic hematoma aspiration （SHPS） combined with iutracerebral henlorrhage （HICH）. [Methods] 42 cases of HICH with SHPS combined with Xingnaojing treatment （case group）, underwent CHR treatment in patients with HICH and 42 cases as control group. [ Results] The cure rate was significantly higher than that in control group （P 〈0.01）; and the case group mortality was also lower than the control group （P 〈0.01）. Compared with the overall efficacy of the two groups, the case group was significantly better than the control group （u =2.7354, P 〈0.01）. Cases of pulmonary infection, alimentary tract hemorrhage, two cases of complications were lower than those of the control group （P 〈0.05）; and the case group the incidence rate of complications was significantly lower than that of the con- trol group （P 〈0.01）. In the case group, patients of self-care were more than those in the control group （P 〈0.05）; the number of patients who cannot take care of themselves was obviously lower than that of the control group （P 〈0.01）. The case group after treatment, GCS score was significantly higher than that in control group （P 〈0.01）, GCS score was significantly higher than the groups before treatment （P 〈0.01）. [ Conclusion] SHPS combined with Xingnaojing Injection in the treatment of HICH can improve the cure rate of HICH, reduce the mortality rate, decrease the complications, improve security, reduce the degrees of coma patients, and reduce the long-term disability rate, compared with the traditional craniotomy advantage.
