摘  要:目的探讨128层CT全脑灌注成像在脑胶质瘤术前分级中的应用价值。方法根据病理检查结果将32例脑胶质瘤患者分为低级别组(Ⅰ~Ⅱ级)21例和高级别组(Ⅲ~Ⅳ级)11例,分别行CT灌注扫描,测量病灶区域和对侧正常脑组织区域的脑血流量(CBF)、脑血容量(CBV)及相对脑血流量(r CBF)、相对脑血容量(r CBV)。结果病灶区域CBF、CBV值明显高于对侧正常区域(P〈0.05);高级别组病灶区CBF、CBV、r CBF、r CBV均明显高于低级别组(P〈0.05);低级别组和高级别组正常区域CBF、CBV值比较,差异无统计学意义(P〉0.05);低级别组CBF、CBV离散程度较高级别组大。结论 128层CT灌注成像在胶质瘤术前分级中有较高的临床应用价值。Objective To evaluate the application of 128 multislice-spiral CT perfusion imaging(CTPI) in preoperative grading of cerebral gliomas. Methods Thirty-two patients with cerebral gliomas were divided into low-grade group (grade Ⅰ - Ⅱ,21 cases) and high-grade group (grade Ⅲ - Ⅳ, 11 eases) according to the pathological results, CT perfusion scans were performed respectively. Cerebral blood flow(CBF) ,cerebral blood volume(CBV) ,relative cerebral blood flow(rCBF) and relative cerebral blood volume(rCBV) of tumor lesions and contralateral normal brain tissues were measured. Results CBF and CBV of tumor lesions were significantly higher than those of contralateral normal brain tissues (P 〈 0.05 ). CBF, CBV, rCBF and rCBV of tumor lesions in the high-grade group were significantly higher than those in the low-grade group ( P 〈 0.05 ). There was no significant difference in the CBF and CBV of normal brain tissues between low-grade group and high-grade group(P 〉 0.05 ). The dispersion degrees of CBF, CBV in the low-grade group were larger than those in the high-grade group. Conclusion 128 Multi-slice spiral CT perfusion imaging is valuable in the application of preoperative grading of glioma.
