摘  要:目的观察急性淋巴细胞白血病(ALL)患者化疗前后外周血中CD4+CD25+调节性T细胞的变化。方法选择26例化疗后完全缓解的ALL患者进行自体对照研究,采用流式细胞术对化疗前后外周血中CD4+CD25+调节性T细胞进行检测,并采用ELISA检测血清中白细胞介素-10(IL-10)、转化生长因子-β(TGF-β)水平,另选择同期体检健康志愿者20例为对照组,进行上述检查。比较对照组与ALL患者化疗前后外周血中CD4+CD25+调节性T细胞的变化。结果 ALL患者化疗前CD4+CD25+调节性T细胞比例为(5.69±1.39)%,明显高于对照组(P<0.05)。ALL患者化疗后CD4+CD25+调节性T细胞比例明显较化疗前降低(P<0.05)。ALL患者化疗后CD4+CD25+调节性T细胞比例与对照组比较,差异无统计学意义(P>0.05)。ALL患者化疗前IL-10和TGF-β表达水平与对照组比较,差异有统计学意义(P<0.01)。ALL患者化疗后IL-10和TGF-β水平与对照组比较,差异无统计学意义(P>0.05)。相关性分析显示,26例患者化疗前IL-10、TGF-β表达水平与CD4+CD25+调节性T细胞比例均呈正相关(P<0.05)。结论 CD4+CD25+调节性T细胞水平在化疗前后具有显著差异,提示其可能与免疫抑制有关,该指标的检测有助于对ALL患者预后及治疗效果的判断。Objective To observe the changes in CD4 + CD25 + regulatory T ceils in peripheral blood of patients with acute lymphoblastic leukemla(ALL) before and after chemotherapy. Methods A self-control study was performed in 26 ALL patients with complete remission after chemotherapy. Flow cytometry was used to detect CD4 + CD25 +regulatory T cells in peripheral blood before and after chemotherapy, and enzyme-linked immunosorbent assay(ELISA) was used to detect the levels of serum interleukin-10(IL-10) and transforming growth factor-β(TGF-β). Twenty healthy volunteers were selected as control group, and they all received the examinations mentioned above. The changes in CD4 + CD25+ regulatory T cells in peripheral blood of ALL patients before and after chemotherapy were compared between two groups. Results The proportion of CD4 +CD25 + regulatory T cells was (5.69 ± 1.39) % in ALL patients before chemotherapy, which was significantly higher than that in the control group (P 〈 0.05). The proportion of CD4+ CD25 + regulatory T cells decreased significantly in ALL patients after chemotherapy in contrast with that before chemotherapy(P 〈0.05). There was no significnat difference in the proportion of CD4+ CD25 + regulatory T cells between ALL patients after chemotherapy and healthy controls(P 〉0. 05). The expression levels of IL-10 and TGF-β showed a significant difference between ALL patients before chemotherapy and healthy controls (P 〈0. 01 ) ,but showed no significant difference between ALL patients after chemotherapy and healthy controls(P 〉0. 05). The correlation analysis revealed that the expression levels of IL-10 and TGF-β were positively correlated with the proportion of CD4 + CD25 + regulatory T cells among 26 patients before chemotherapy ( P 〈 0.05). Conclusion The levels of CD4 + CD25 + regulatory T cells before and after chemotherapy show a significant difference, which might be associated with immunologic suppression. Its
