孤立性纤维性肿瘤的临床病理特征及预后分析

摘　　要：目的探讨孤立性纤维性肿瘤(SFT)的临床表现、影像学特点、病理学特征及相关预后,并研究紧密连接相关蛋白在SFT中的表达特点。方法回顾性分析20例SFT的临床表现、影像学特点、病理组织学特征、免疫表型及随访结果,并加做紧密连接相关蛋白claudin-1,claudin-3,claudin-4,claudin-7和ZO-1的免疫组织化学检测。结果20例SFT发生部位各异,临床表现也各不相同,组织学形态多样,免疫组化:20/20CD34(+),18/19CD99(+),19/20bcl-2(+),2/19S-100弱(+),1/12EMA局灶(+),2/18SMA局灶(+),7/12claudin-7局灶弱(+),5/12ZO-1局灶弱(+),12例claudin-1、claudin-3、claudin-4均(-)。随访2例复发,其中1例死亡。结论 SFT有细胞分布疏密不均及多态性生长方式的组织学特征。CD34、bcl-2、CD99等有助于病理诊断,紧密连接相关蛋白claudin-1、claudin-3、claudin-4在SFT中表达为阴性,claudin-7和ZO-1部分表达为阳性。SFT患者有必要长期随访。Objective To investigate the clinical manifestation,imaging features,pathological characteristics,and prognosis of the solitary fibrous tumor and investigate the expressive characteristics of tight junction associated proteins in SFT.Methods The features of the clinical manifestation,imaging features,pathological characteristics,immunophenotype and prognosis were analyzed retrospectively in 20 cases of SFT.Immunohistochemistry was performed to detect the expression of claudin-1,claudin-3,claudin-4,claudin-7and ZO-1.Results Twenty cases were different of position,clinical symptom,and histological morphology.Immunophenotype showed 20/20CD34（＋）,18/19CD99（＋）,19/20bcl-2（＋）,2/19S-100（＋）,1/12EMA（＋）,2/18SMA（＋）,7/12claudin-7（＋）,5/12ZO-1（＋）;claudin-1,claudin-3and claudin-4were negtive in 12 cases.Two patients recurrented and one of them died.Conclusion SFTs are composed of juxtaposed hyper-and hypo-celluar cells and showed different histological features.CD34,bcl-2,and CD99 are helpful to the diagnosis of the SFT.Claudin-1,claudin-3and claudin-4are negtive,but claudin-7and ZO-1maybe positive in SFT.Long-term clinical follow-up is necessary for the patients with SFT.
