甲状腺癌患者血清Hpa、uPA检测及其临床意义

摘　　要：目的检测甲状腺癌患者血清乙酰肝素酶(Heparanase,Hpa)和尿激酶型纤溶酶原激活剂(Urokinase-type plasminogen activator,u PA)含量,探讨其在甲状腺癌诊断中的价值。方法采用酶联免疫吸附测定法(ELISA)测定38例甲状腺癌患者,37例良性甲状腺结节患者和30例健康人对照组的血清Hpa和u PA水平,并比较各组之间的差异。结果甲状腺癌组Hpa、u PA浓度水平明显高于其他两组,其差异均有显著统计学意义(P<0.01),而良性甲状腺结节组与正常对照组比较则差异无统计学意义(P>0.05)。血清中Hpa浓度与淋巴结转移与否差异无统计学意义(P>0.05),而淋巴结转移组血清中的u PA浓度明显高于无淋巴结转移组,差异有统计学意义(P<0.05)。Hpa/u PA的曲线下面积为0.812,联合检测对甲状腺癌的诊断价值大于Hpa和u PA单独检测(P<0.05)。结论血清Hpa和u PA联合检测在甲状腺良恶性肿瘤的鉴别和预测是否有淋巴结转移方面有一定的临床价值。Objective To explore the clinical significance of serum Hpa and uPA detection in patients with thyroid Carcinoma. Methods Enzyme linked immunosorbent assay（ELISA） was applied to detect the expression of Hpa and uPA in serum of 38 cases of thyroid carcinoma（thyroid carcinoma group） and 37 cases of patients with benign thyroid nodules（benign thyroid nodules group）, 30 normal persons（the control group）. Results Hpa and uPA levels in thyroid carcinoma group were significantly higher than those in other two groups（P〈0.01）, but there was no statistically significant difference between benign thyroid nodules group and the control group（P〉0.05）. The expression of Hpa showed no significant difference between the patients with lymph node metastasis and those without lymph node metastasis（P〉0.05）, but the expression of uPA in patients with lymph node metastasis was significantly higher than those without lymph node metastasis（P〈0.05）. The area under curve（AUC） of the combination of Hpa/uPA was 0.812, which was significantly higher than that of Hpa or uPA alone（both P〈0.05）. Conclusion The combined detection of Hpa and uPA may have certain application value for the early diagnosis of thyroid carcinoma.
