摘 要:目的探讨基于局部一致性(ReHo)、低频振幅(ALFF)和低频振幅分数(fALFF)的静息态功能磁共振成像(RS-fMRI)是否能一致、互相补充地发现颞叶内侧癫痫(mTLE)患者局部异常脑活动区域。资料与方法 20例mTLE患者与20例年龄、受教育程度相匹配的正常人行RS-fMRI检查,对经ReHo、ALFF和f ALFF分析获得的静息态数据进行比较。结果 mTLE患者左侧颞叶和中央前、后回较对照组ReHo值增高(t=4.42、4.12、4.00,P<0.001),双侧额叶皮层下白质较对照组减低(t=-3.82、-3.75,P<0.001);mTLE患者右侧扣带回ALFF值较对照组增高(t=3.70,P<0.001),右侧额叶和岛叶ALFF值较对照组减低(t=-4.14、-3.92,P<0.001);mTLE患者左侧颞叶和中央前回f ALFF值较对照组增高(t=5.19、5.90,P<0.001),双侧额叶f ALFF值较对照组减低,以左侧为著(t=-5.85、-4.27,P<0.001)。结论 mTLE患者在ReHo、ALFF及f ALFF分析中表现出较广泛并几乎一致的局部异常脑活动区域,脑活动增加的区域具有左侧优势,提示ReHo、ALFF及f ALFF可以用于癫痫病理生理机制和致痫灶定侧、定位的研究。Purpose To investigate whether resting state functional MRI (RS-fMRI) based on regional homogeneity (ReHo), amplitude of low-frequency fluctuation (ALFF) and fraction of ALFF (fALFF) can reveal brain regional functional abnormalities in patients with mesial temporal lobe epilepsy (mTLE) consistently and complementarily. Materials and Methods RS-fMRI were performed on 20 patients with mTLE and 20 healthy volunteers who were age and education background matched, MRI data was compared and analyzed using ReHo, ALFF and fALFF analysis. Results mTLE patients showed increased ReHo in left temporal lobe, precentral gyrus and postcentral gyrus than normal controls (t=-4.42, 4.12 and 4.00, P〈0.001) and decreased ReHo in bilateral subfrontal white matter (t= - 3.82 and - 3.75, P〈0.001); increased ALFF was found in right cingulated cortex (t=3.70, P〈0.001) and decreased ALFF was found in right frontal lobe as well as insula lobe (t=- 4.14 and -3.92, P〈0.001). Increased fALFF was found in left temporal lobe and precentral gyrus (t=5.19 and 5.90, P〈0.001) and decreased fALFF was found in bilateral frontal lobes with left side dominance (t=-5.85 and -4.27, P〈0.001). Conclusion mTLE patients showed widely-distributed and almost the same differences in ReHo, ALFF and fALFF with left side dominance in brain activity increased area, the combination using of them may yield a more comprehensive pathophysiological framework, as well as lateralization and localization ofepileptogenic zone for mTLE.
