摘  要:目的总结分析原发性喉鳞状细胞癌及其相邻正常黏膜组织新鲜标本中cyclinG1、G2蛋白表达与喉癌的组织病理分级、临床分期以及解剖分型的关系,以探讨cyclinG1、G2蛋白表达与喉癌的可能相关性。方法回顾性分析52例喉癌(喉癌组)及其相邻正常黏膜组织标本(手术切缘癌旁1.0cm外黏膜组织,对照组)的临床病理资料,52例喉癌组织标本经病理切片确诊。所有患者术前均未行放疗和化疗,均无远隔器官转移。术后病理TNM分期:Ⅰ期8例(T1N0M0),Ⅱ期12例(T2N0M0),Ⅲ期24例(T3N0M06例,T3N1M08例,T2N1M010例),Ⅳ期8例(34N0M01例,T4N0M0l例,T3N2M03例,T2N0M03例)。病理分级:G1高分化鳞癌21例,G2中分化鳞癌18例,C3低分化鳞癌13例。声门型28例,声门上型24例,无声门下型病例。采用免疫组化(SP)法检测组织中cyclinG1、G2蛋白表达。结果cyclinG1蛋白表达阳性部位位于细胞核,而cyclinG2位于细胞质中。喉癌及其相邻正常黏膜组织中cyclinG1蛋白表达阳性百分比分别为82.69%(43/52)和30.77%(16/52),cyclinG2分别为21.15%(11/52)和61.54%(32/52)。与相邻正常黏膜组织比较,cyclinG1蛋白在喉癌中表达增高,而cyclinG2蛋白表达降低,差异均具有统计学意义(P〈0.01)。喉癌的组织病理分化越低、临床分期越高,cyclinG1蛋白表达增高越明显,cyclinG2蛋白表达降低越明显。cyclinG1、cyclinG2蛋白表达与喉癌的解剖分型无相关性。结论喉癌中cyclinG1蛋白表达增高,cyclinG2蛋白表达降低,提示其可能与喉癌的发生、发展机制有关。cyclinG1、G2蛋白的检测可望成为判断喉癌预后的指标之一。Objective To investigate the expression of cyclin G1 and G2 within laryngeal squamous cell carcinoma (LSCC) and their surrounding normal laryngeal mucosa and their potential relationship with the pathological grade, clini- cal stage and anatomic type of LSCC. Methods Clinical data from 52 LSCC samples and their corresponding normal mu- cosa (external mucosa 1.0 cm from the surgical margin) were retrospectively analyzed. All LSCC samples were pathologi- cally determined. All patients did not receive chemotherapy or radiotherapy before surgery. They did not undergo remote organ transplantation. After surgery, the pathological TNM were staged as follows: 8 samples with stage I (T1N0M0) , 12 with stage II (T2N0M0), 24 with stage Ill (including 6 with T3 No M0, 8 with T3 N1 M0 and 10 with T2 N1 M0 ), and 8 with stage IV ( including 1 with T4N0M0, 1 with T4N1M0, 3 with T3N2Mo and 3 with T2N2M0). The pathological grades were as follows: 21 samples with G1 advanced differentiated LSCC, 18 with G2 moderately differentiated LSCC, and 13 with G3 low differentiated LSCC. Among these LSCC samples were 28 with glottic carcinoma and 24 with supraglottic car- cinoma, without subglottic ones. The expressions of cyclin G1 and G2 protein were determined by immunohistochemical ( S - P) method. Results The positive expression of cyclin G1 protein was found within the nucleus, while cyclin G2 was present in the cytoplasma. The positive percentage of cyclin G1 expression was 82.69% (43/52) in LSCC and 30.77% (16/52) in their surrounding normal laryngeal mucosa. Meanwhile, the positive percentage of cyclin G2 expression was 21.15% (11/52) in LSCC and 61.54% (32/52) in their surrounding normal laryngeal mucosa. Compared with the normal samples, increased amounts of eyelin G1 but declined levels of eyclin G2 were detected in LSCC, with stat/stieal difference (P 〈 0.01 ).
