摘　　要：目的分析氟代脱氧葡萄糖(18F-FDG)在原发性胃淋巴瘤(PGL)的摄取特点,探讨PET-CT显像对其诊断价值。方法回顾性分析53例经病理确诊为PGL患者(PGL组)的18F-FDG PET-CT图像及临床资料,分析病灶最大标准摄取值(SUVmax)与D-二聚体、乳酸脱氢酶的关系。同期收集50例胃癌患者(胃癌组)、45例健康查体者(查体组)的PET-CT显像结果,对比分析PGL的PET-CT显像特点。结果胃癌组、查体组与PGL组SUVmax比较,差异均有统计学意义(P均<0.05);黏膜相关性PGL、胃癌与侵袭性PGL患者SUVmax比较差异有统计学意义(P均<0.05)。在PGL患者中,不同病理分型、临床分期间SUVmax差异有统计学意义(P均<0.05);D-二聚体水平与SUVmax显示较好相关性(r=0.831,P<0.01)。结论临床确诊或可疑PGL患者及时行PET-CT检查,结合D-二聚体水平,有利于其鉴别诊断、临床分期及治疗方案的确定。Objective To investigate the uptake characteristics of ^18F-FDG in primary gastric lymphoma（ PGL）,and to investigate the diagnostic value of ^18F-FDG positron emission tomography-computed tomography（ PET-CT） imaging.Methods We studied the clinical data and ^18F-FDG PET-CT imaging of 53 cases of patient with PGL（ PGL group）.The relationship between the SUVmax value and the relevant clinical indexes was analyzed.PET-CT imaging results were collected from 50 cases of gastric cancer（ gastric cancer group） and 45 cases of normal physical examination（ control group）,and then the comparative analysis of PET-CT imaging features of PGL was conducted.Results The SUVmax of gastric cancer group and control group was significantly different as compared with that of PGL group（ all P〈 0.05）; and the SUVmax of mucosa associated gastric lymphoma group and gastric cancer group was significantly different as compared with that of the aggressive primary gastric lymphoma（ all P〈 0.05）.The SUVmax of PGL in different pathologic classification and clinical stage groups was significantly different; D- dimer level and SUVmax showed a good correlation（ r = 0.831,P〈 0.01）.Conclusion The patients clinically diagnosed or suspected with PGL should receive PET-CT imaging combined with relevant clinical index detection in time,which is important in the differential diagnosis,clinical stages and therapy of primary gastric lymphoma.
