摘  要:目的:讨论超声、甲胎蛋白(alpha fetoprotein,AFP)及计算机断层扫描(computed tomography,CT)对原发性肝癌的诊断价值.方法:2000-01/2008-12宁夏医科大学总医院肝胆外科共收治经超声、CT及AFP诊断并经手术病理或临床证实的原发性肝癌208例.208例经手术病理或临床证实的原发性肝癌,全部病例均作了B超、AFP及CT检查.根据超声及CT所见将其分为三型:(1)巨块型;(2)结节型;(3)弥漫型.结果:AFP检查:阳性124例,敏感性为59.6%.AFP检查阴性84例(40.4%).超声共检出肝癌病灶186例,敏感性为89.4%,其中162例做出定性诊断(87.1%),24例提示肝内占位性病变.漏误诊22例(10.6%).CT检查:共检出肝癌病灶190例,敏感性为91.3%,其中做出定性诊断170例(89.4%),未予定性20例.漏误诊18例(8.7%).结论:如果3种方法联合使用,可提高肝癌的检出率.AIM: To assess the diagnostic value of ultrasound, alpha fetoprotein (AFP) and computed tomography (CT) for primary liver cancer. METHODS: Two hundred and eight patients with pathologically or clinically confirmed primary liver cancer who were treated and underwent ultrasound, CT and AFP examinations at Ningxia Medical University General Hospital from January 2000 to December 2008 were included. According to ultrasound and CT findings, primary liver cancer could be classified into 3 types: (1) huge mass type; (2) nodular type; and (3) diffuse type.RESULTS: AFP evaluation revealed 124 positive cases and 84 (40.4%) negative cases, with a sensitivity of 59.6%. Ultrasound detected liver lesions in 186 cases (sensitivity, 89.4%), of which 162 (87.1%) achieved a diagnosis (including 24 cases with space occupying liver lesions) and 22 (10.6%) had a misdiagnosis. CT examination detected liver lesions in 190 cases (sensitivity, 91.3%), of which 170 (89.4%) achieved a diagnosis, 20 had undetermined nature of lesions, and 18 (8.7%) had a misdiagnosis. CONCLUSION: Combination of the three modalities may yield better detective results.
