摘 要:目的探讨不同角膜曲率对泪膜稳定性的影响。方法临床病例对照研究。对2011年4月至2012年5月在延边眼耳鼻喉科医院眼科的健康体检正常者216人(216只眼),根据角膜曲率大小分为三组:≤43D组、43D〈K〈45D组以及≥45D组。分别测量泪膜破裂时间(breakuptime,BUT)、泪液分泌试验(SchirmerItest,SIt)及眼前节光学相干断层扫描(anteriorsegmentopticalcoher.encetomography,AS—OCT)测量泪河相关参数,进行统计学分析。结果三组之间BUT值差异具有统计学意义(F=3.572,P=0.031),但三组之间SIt值差异无统计学意义(F=0.803,P=0.450)。角膜曲率和BUT值之间差异有统计学意义(r=-0.201,P〈O.05),而角膜曲率与SIt以及泪河相关参数之间差异无统计学意义(P〉0.05)。结论角膜曲率越大,其BUT值越小,陡峭的角膜前表面可导致泪膜稳定性下降。Objective To discuss the effects of different central corneal curvature on the stability of tear film. Methods The study included healthy subjects 216 eyes of 216 cases in the hospital, and were divided into the following 3 groups according to K-value: ≤43D, 〉43D and 〈45D, and ≥45D. Tear film breakup time (BUT), Schirmer I test (S I t), and tear meniscus related param- eters were measured by anterior segment optical coherence tomography (AS-OCT), on which the da- ta were statistical analyzed. Results BUT value discrepancy of three groups had statistical signifi- cance (F =3.572, P =0.031), but S I t value discrepancy of three groups had no statistical signifi- cance (F =0.803, P =0.450). K-value was significantly negative correlated with BUT (r =-0.201, P 〈 0.05). The K-value was not significantly correlated with S I t score or tear meniscus parameters (P 〉 0.05). Conclusions K-value is significantly negative correlated with BUT, steep anterior corneal surface may result in decreased tear film stability.
