下直肌后退术对甲状腺相关眼病患者眼压的影响

摘　　要：目的研究下直肌后退术对甲状腺相关眼病（thyroid．associatedophthalmopathy，TAO）患者眼压的影响。方法：临床病例对照研究。对2003年10月1日至2013年4月30日因TAO限制性下斜视在深圳市眼科医院行单纯下直肌后退术患者的临床资料进行回顾分析。22例26只眼符合人选标准并被纳入研究。主要观察指标为术前和术后1d、3月的眼压值。根据术前眼压将患者分为正常眼压组和高眼压组。结果：高眼压组患者行下直肌后退术后的眼压与术前相比明显降低，术前、术后1d、3月的眼压均值分别为（27．92±4．55）mmHg、（18．81±3．96）mmHg（P=0．000）和（15．96±1．78）mmHg（P=0．004）。而正常眼压组患者行下直肌后退术后的眼压与术前相比无明显差异，术前、术后1d、3月的眼压均值分别为（16．78±2．87）mmHg、（15．52±1．93）mmHg（P=0．419）和（14．81±1．47）mmHg（P=0．298）。结论：下直肌后退术能够在术后早中期降低TAO患者因限制性下斜视引起的高眼压。Objective To study the affect of inferior rectus recession on intraocular pressure （lOP） in patients with thyroid-associated ophthalmopathy. Methods The medical records of patients with restrictive hypotropia secondary to thyroid-associated ophthalmopathy who underwent inferior rec- tus recession from October 1, 2003 through April 30, 2013 were reviewed and analyzed retrospective- ly. Twenty-two patients （26 eyes） met the criteria and were included in this study. The main out- come measurement was lOP readings obtained before and after operation 1 day and 3 months. The patients were divided into ocular hypertension group and normal IOP group according to preopera- tive IOP readings. Results A statistically significant decrease in IOP in ocular hypertension group was noted after inferior rectus recession. The mean IOP in ocular hypertension group before and af- ter operation 1 day and 3 months was 27.92±4.55mmHg, 18.81±3.96 mmHg and 15.96±1.78 mmHg. The mean IOP in normal IOP group before and after operation 1 day and 3 months was 16.78±2.87 mmHg, 15.52±1.93 mmHg and 14.81±1.47 mmHg. Conclusions Inferior rectus recession result in a significant reduction in IOP in the early and medium postoperative period in ocular hypertension patients with restrictive myopathy secondary to thyroid-associated ophthalmopathy.
