摘 要:目的研究高度近视与中低度近视患者周边屈光度的差异。方法对45例2013年6~10月在本院眼视光学中心就诊的近视患者(-2.50^-11.50 D)进行周边屈光度测量。患者平均年龄为(28.67±7.37)岁(20~47岁)。将患者分为高度近视组(球镜>-6.00 D,26例)和中低度近视组(球镜≤-6.00 D,19例)。遮盖左眼,右眼裸眼状态下,从颞侧30°至鼻侧30°依次注视7个2.5 m远视标,用红外自动验光仪获取6个角度的周边屈光度和1个中心屈光度。周边屈光等效球镜值(Ma)减去中心屈光等效球镜值(M0)即得到相对周边屈光度(RPRE)。周边与中心散光均分解为180°轴位散光(J180)和45°轴位散光(J45)。结果高度近视组和中低度近视组的平均RPRE在颞侧视网膜差异有统计学意义(P<0.05),且随偏中心注视角度的增加而增大,高度近视组的RPRE比中低度近视组更倾向于远视性离焦,而在鼻侧视网膜差异无统计学意义(P值均>0.05)。高度近视组鼻、颞侧RPRE不对称,颞侧视网膜较鼻侧视网膜远视性离焦量大(P<0.05)。2组患者周边散光差异无统计学意义。结论高度近视颞侧远视性离焦较中低度近视更加显著,可能与高度近视患者近视持续加深相关。Objective To compare the difference of peripheral refraction between patients with high myopia and with low-moderate myopia.Methods Forty-five patients with myopia (-2.50±11.50 D) were included in the study. The mean age was 28.67 ±7.37 years old (ranging from 20 to 47 years old).The patients were divided into high myopia group (sphere〉-6.00 D, n=26) and low-moderate myopia group (sphere ≤-6.00D, n=26).An open-field auto-refractor was used to measure on-and off-axis refractions in uncorrected state .Peripheral refractions were measured along 10 degrees, 20 degrees and 30 degrees from central fixation in both nasal and temporal fields .Refractive data were transposed into M, J180 and J45 vectors for analysis.The difference between peripheral refraction and central refraction was described as relative peripheral refractive error ( RPRE ) .Results There was a significant difference in RPRE of temporal retina only(P〈0.05) between the two groups, and relatively more hyperopic sphere was showed in the high myopia group.In addition, the RPREs were asymmetric in nasal-temporal retina in the high myopia group (P〈0.05). There was no significant difference of peripheral astigmatism between the two groups .Conclusions The RPRE of the high myopia group is more hyperopic than that of the low-moderate myopia group in the temporal retina , which may be correlated to the progression of myopia in the high myopia patients .
