糖尿病性黄斑水肿患者视网膜黄斑区形态与视觉功能的相关性

摘　　要：目的研究糖尿病性黄斑水肿患者视网膜黄斑区形态与视觉功能的相关性。方法收集125例糖尿病性视网膜病变患者共157眼,根据病变程度分为60例72眼设为黄斑水肿组,65例85眼设为非黄斑水肿组,另选择同期健康者30例60眼作为正常对照组,观察三组最佳校正视力(BCVA)、视网膜厚度、视网膜光敏度。结果三组BCVA差异有统计学意义(P<0.05)。三组黄斑中央区、上侧、下侧、鼻侧、颞侧5个象限的视网膜厚度差异均有统计学意义(P<0.05)。三组黄斑中央区、上侧、下侧、鼻侧、颞侧5个象限的视网膜光敏度差异均有统计学意义(P<0.05)。视网膜厚度均数与BCVA呈显著正相关(P<0.05),黄斑中央区、上侧、下侧、鼻侧、颞侧厚度与光敏度分别呈显著负相关性(P<0.05)。BCVA的ROC曲线下面积为0.823(P<0.05),视网膜光敏度的ROC曲线下面积为0.879(P<0.05)。视网膜光敏度对黄斑区形态改变导致视觉受损评估价值更高(P<0.05)。结论糖尿病性黄斑水肿患者黄斑区形态与视觉功能密切相关,分象限视野检查对判断视觉功能受损较BCVA更优越,可更准确反映黄斑区形态和功能改变。Objective To study the correlation of diabetic macular edema in patients with retinal macular morphology and visual function. Methods A total of 125 cases of diabetic retinopathy patients with 157 eyes, were divided into severity of macular edema groups of 60 cases with 72 eyes, and non-macular edema group of 65 cases with 85 eyes,chose another 30 cases of 60 healthy subjects in the same period as the normal control group, the best corrected visual acuity（BCVA）, retinal thickness, retinal photosensitivity of three groups were observed. Results The BCVA among three groups were significantly（P〈0.05）. Three central macular area, upper, lower, nasal, temporal retinal thickness difference five quadrants were statistically significant（P〈0.05）. Three central macular area, upper, lower, nasal, temporal differences in retinal photosensitivity five quadrants were statistically significant（P〈0.05）. Retinal thickness and mean BCVA were significant positive correlation（P〈0.05）, central macular area, upper, lower, nasal, temporal thickness and photosensitivity were significant negative correlations（P〈0.05）. Area under the ROC curve BCVA was 0.823（P〈0.05）, the area under the ROC curve of retinal light sensitivity of 0.879（P〈0.05）. Light sensitivity of the retina macular morphological changes lead to higher assessed value for visually impaired（P〈0.05）. Conclusion Patients with diabetic macular edema is closely related macular morphology and visual function, compared to determine BCVA, sub-quadrant perimetry visual impairment is superior, can more accurately reflect the morphological and functional changes in the macula.
