摘 要:目的评价超声造影、声触诊组织成像(VTI)及声触诊组织量化(VTQ)对乳腺不典型病灶良恶性的鉴别诊断价值。方法对261例307个乳腺不典型病灶行彩色多普勒超声、VTI、VTQ及超声造影检查,结果与术后病理对照分析。结果 307个乳腺不典型病灶中,良性186个,恶性121个。VTI诊断恶性病灶的敏感性为66.9%,特异性为41.4%,诊断准确率51.1%;VTQ诊断恶性病灶的敏感性为83.5%,特异性为88.2%,诊断准确率90.6%;超声造影诊断恶性病灶的敏感性为85.1%,特异性为89.2%,诊断准确率87.6%;三者联合应用诊断恶性病灶的敏感性为94.2%,特异性为95.7%,诊断准确率93.8%。VTI与VTQ和超声造影比较差异有统计学意义(P<0.05);VTQ与超声造影比较差异无统计学意义。结论超声造影、VTI及VTQ诊断乳腺不典型病灶的良恶性各有优势,三者联合应用可进一步提高诊断准确率,为临床治疗提供较为可靠的依据。Objective To evaluate the diagnostic value of differentiating benign from malignance of breast lesions by contrast- enhanced ultrasound(CEUS), virtual touch tissue imaging(VTI) and virtual touch tissue quantification(VTQ).Methods A total of 307 breast lesions among 261 patients were respectively examined by color Doppler sonography,VTI,VTQ and CEUS. The results were compared with the pathological results after operation. Results Among 307 breast lesions,186 were benign and 121 were malignant. The sensitivtiy and specificity of VTI were 66.9% and 41.4%,the accuracy was 51.1%. The sensitivity and specificity of VTQ were 83.5% and 88.2%,the accuracy was 90.6%. The sensitivity of CEUS was 85.1%,the specificity was 89.2%,and accuracy was 87.6%. The sensitivity, specificity and accuracy of the combination with three technologies were 94.2%,95.7% and 93.8%, respectively. The differences among VTI,VTQ and CEUS were statistically significant(P〈0.05),but the difference between VTQ and CEUS was not statistically significant. Conclusion Combining with CEUS,VTI and VTQ application could improve the differential diagnosis of breast lesions.
