肺淋巴管平滑肌瘤病中β-catenin的表达及鉴别诊断意义

摘　　要：目的探讨β-catenin在肺淋巴管肌瘤病(PLAM)中的表达及其在鉴别诊断中的意义。方法采用免疫组化检测PLAM、正常肺组织和间质性肺炎平滑肌增生各10例,朗格汉斯组织细胞增生症、肾血管平滑肌脂肪瘤和血管球瘤各6例以及4例血管周细胞瘤中β-catenin的表达。结果β-catenin在PLAM中阳性率为100%,平均评分7,细胞质/膜弥漫(+);在血管平滑肌脂肪瘤中阳性率为67%,平均评分3.2,细胞质强弱不等颗粒状(+);在间质性肺炎平滑肌增生、血管球瘤、郎格汉斯组织细胞增生症中阳性率分别为0、33.3%和0,与PLAM比较差异显著(P<0.05)。β-catenin在血管周细胞瘤中阳性率75%,平均评分3.8,细胞核旁点状(+)。结论β-catenin可与HMB45联合应用于PLAM的诊断,并作为PLAM鉴别诊断指标。Objective To investigate expression of β-catenin in pulmonary lymphangioleiomyomatosis and discuss their significance. Methods Expression of β-catenin were examined in 10 pulmonary lymphangioleiomyomatosis,10 normal lung tissues, 10 interstitial pneumonia with smooth muscle hyperplasia, 6 langerhans cell histiocytosis, 6angioleiomyolipoma,6 glomus tumor and 4 hemangiopericytoma using immunohistochemistry. Results The positive expression rate of β-catenin in pulmonary lymphangioleiomyomatosis was 100%（ 10 /10） and average score was 7. Diffuse positive stain fixed on membrane and cytoplasm. The positive expression rate of β-catenin in angioleiomyolipoma was 67%（ 4 /6） and average score was 3. 2. Inconsistent stain fixed in cytoplasm. The positive expression rate of β-catenin in interstitial pneumonia with smooth muscle hyperplasia,glomus tumor and Langerhans cell histiocytosis was 0（ 0 /10）,33. 3%（ 2 /6） and 0（ 0 /6）,respectively which were lower than that in pulmonary lymphangioleiomyomatosis（ P〈0. 05）.The positive expression rate of β-catenin in hemangiopericytoma was 75%（ 3 /4） and average score was 3. 8. The positive stain fixed in cytoplasm near the nuclear. Conclusion β-catenin is a useful marker in pulmonary lymphangioleiomyomatosis combined with HMB45. It is useful to distinguish pulmonary lymphangioleiomyomatosis from interstitial pneumonia with smooth muscle hyperplasia,Langerhans cell histiocytosis,angioleiomyolipoma,glomus tumor and hemangiopericytoma.
