摘  要:目的研究电针对脑卒中后抑郁(PSD)患者局部脑血流量(r CBF)的影响。方法将21例脑卒中后抑郁患者随机分为电针组(n=11)和药物组(n=10)。电针组取双侧合谷、太冲穴,每天1次;药物组口服盐酸氟西汀分散片20 mg,每天1次,均连续治疗8周。患者分别于治疗前及治疗后2、4、8周使用汉密尔顿抑郁量表(HAMD)进行评定,治疗前及治疗后8周采用单光子发射计算机断层成像(SPECT)观察r CBF的变化。结果电针组治疗后2周时HAMD低于药物组(P<0.05),而治疗后4周、8周时无显著性差异(P>0.05)。治疗8周后,电针组r CBF较药物组改善更佳(P<0.05)。结论电针治疗脑卒中后抑郁疗效明确,可能与改善卒中后脑灌注有关。Objective To explore the therapeutic effects of electroacupuncture on regional cerebral blood flow(r CBF) in patients with post-stroke depression. Methods 21 patients with post-stroke depression were randomly divided into electroacupuncture group(n=11) and drug group(n=10). The electroacupuncture group received electroacupuncture on double Hegu(LI4) and Taichong(LR3) once a day for 8weeks, while the drug group took fluoxetine hydrochloride dispersible tablets 20 mg a day for 8 weeks. They were evaluated with Hamilton Self-rating Scale for Depression(HAMD) before, and 2 weeks, 4 weeks and 8 weeks after treatment respectively. The r CBF were measured with single positron emission computerized tomography(SPECT) before and 8 weeks after treatment. Results The score of HAMD decreased more in the electroacupuncture group than in the drug group 2 weeks after treatment(P〈0.05), and there was no significant difference between the two groups 4 and 8 weeks after treatment(P〉0.05). The r CBF was significantly greater in the electroacupuncture group than in the drug group. Conclusion That electroacupuncture is effective on post-stroke depression, which may be related with the increase of the r CBF levels.
