摘  要:目的通过葡萄糖渗漏法和micro-CT三维扫描评价iRoot SP根管三维充填的封闭能力。方法单根管下颌前磨牙55颗截去牙冠保留牙根12 mm,机用镍钛预备根管至30#,按随机数字表法将标本分为3个实验组(A组:iRoot SP直接注入根管充填;B组:iRoot SP+热牙胶充填;C组:AH Plus+热牙胶充填。n=15)和2个对照组[阴性对照组(D组)、阳性对照组(E组),n=5]。建立微渗漏葡萄糖定量分析模型,分别于第1天,第1、2、3、4、5、6周用葡萄糖氧化酶法检测从牙冠向牙根渗出的葡萄糖浓度。同时micro-CT三维扫描分析充填根管内存在的间隙。结果在前4周,3个实验组平均每天微渗漏量<0.18 mmol/L,几乎没有微渗漏;第5、6周时,C组平均微渗漏值>0.18 mmol/L,有微渗漏发生,且C组与A、B组之间微渗漏量差异有统计学意义(P<0.05)。micro-CT三维扫描发现:A组根管中上段无间隙,根尖1 mm见有间隙形成。B、C组根管内有间隙形成。结论 iRoot SP用于根管三维充填,具有良好的封闭性,而且一元相充填操作简单,对牙周组织没有损伤,对形成根管三维充填有一定的优势。Objective To evaluate the sealing ability of iRoot SP( a bioceramic material) in root canal obturation with a quantitative method using glucose and micro-computed tomography( micro-CT).Methods Fifty-five mandibular premolars with single root canal were decoronated to obtain 12-mm root segments. The root canals were instrumented using Protaper Files prepared to 30 #. The roots were randomly divided into 3 experimental groups A,B and C( n = 15),a negative control group D( n = 5),and a positive control group E( n = 5). The root canals of group A were directly filled with iRoot SP without a gutta-percha point. The root canals of group B were filled with iRoot SP using continuous wave of condensation technique.The root canals of group C were filled with AH Plus( root canal sealer) using continuous wave of condensation technique. Models were built to measure leakage with the quantitative method using glucose. The concentration of glucose leaked from the crown to the root was measured with a glucose oxidase-peroxidase method on the 1st day and in the 1st,2nd,3rd,4th,5th and 6th weeks during the experimental period.Voids in the root canal fillings were assessed using cross-section images from micro-CT scanning. Results During the 1st to 4th weeks,there was almost no glucose leakage in the 3 experimental groups,and the average daily leakage concentration was less than 0. 18 mmol / L. In the 5th and 6th weeks,there was glucose leakage( 0. 18 mmol / L) in group C,and there were significant differences between group C and group A,as well as differences between group C and group B( P〈 0. 05). Micro-CT scanning found that voids were formed only at the apical part( 1 mm) in group A,while voids are formed in the root canals in group B and group C. Conclusion iRoot SP as cement paste has excellent sealing ability in root canal three-dimensional obturation,and can realize primary monoblocks without damage to periodontal tissue.
