摘  要:目的 观察绝经年龄对老年女性颈动脉斑块及脑白质损害(WML)发生的影响并探讨其机制.方法 研究对象来自本院健康体检年龄≥65岁的老年女性.选择绝经年龄<46岁、47-49岁及≥50岁老年女性各50人,所有研究对象行常规临床检查、颈部血管超声及头颅MRI检查.单因素方差分析及x2检验比较各组临床资料的差异,Logistic回归分析绝经年龄对发生颈动脉斑块及WML风险值的影响.结果 绝经年龄<46岁组的体质量、血压、血脂异常、颈动脉斑块及WML的比率较绝经年龄≥50岁组明显增加(P<0.05),调整混杂因素后早期绝经组发生颈动脉斑块(OR=2.73,%95 CI:1.13~ 6.26)及WML(OR=2.46,%95 CI:1.14~5.85)的风险明显增加.结论 早期绝经可加重颈动脉及脑部小血管的动脉硬化.Objective To examine the association of age at menopause with risk of the carotid plaque and cerebral white matter lesions(WML) and explore the underlying mechanisms. Methods Participants were women aged 65 and over who came to perform heahh examination in Jinling hospital.The participants were divided into three groups:women with menopause at ages 〈46 (n = 50), with menopause at ages 47 to 49 (n = 50) and with menopause at ages 1〉 50 (n = 50). All participants underwent cranial MRI scan and carotid ultrasound examination.A comparison of clinical characteristics was made among the menopause age groups. Logistic regression was used to assess odds ratios (ORs) and 95% confidence intervals (CIs) for carotid plaque and WML associated with age at menopause. Results Women with menopause at ages 〈46 years had higher body-mass index, higher percentage of hypertension and serum lipid abnormality compared with those with menopause ages 1〉 50, menopause before age 46 was associated with increased risk of carotid plaque ( OR = 2.73,95% CI: 1.13 - 6.26) and WML ( OR= 2.46,95% C[: 1.14 - 5.85 ) adjusted for BMI, hypertension and serum lipid abnormality compared with those menopause ages I〉 50. Conclusions Early menopause accelerates atherosclerosis of both carotid artery and cerebral small vessels.
