由神经血管单元概念审视假性延髓麻痹二例

摘　　要：目的观察依据神经血管单元概念自订方案治疗脑梗死后假性延髓麻痹2例患者的临床疗效。方法选择有高血压、糖尿病共患疾病2例脑梗死患者,发病后即出现假性延髓麻痹,在其病后2w,遵照神经血管单元概念,由消栓通颗粒3.0·d-1,异山梨醇口服液60ml·d-1,硫辛酸注射液0.6·d-1(除去胃管后改用硫辛酸胶囊0.6·d-1加用苏子油软胶囊3.0·d-1)组成的治疗方案,观察除去鼻饲时间、正常进食时间及1年后随访情况。结果 2例患者分别于2 w后取出胃管,1个月后正常进食,无药物不良反应。1年后随访无复发,生活自理。结论依据神经血管概念自订治疗脑梗死后假性延髓麻痹的方案是合理的、有效的,但需大宗病例临床研究进一步证实。Objective To observe the clinical effects of the custom solutions on the basis of neurovascular unit in treating the two cases of pseudobulbar paralysis after cerebral infarction.Methods The two cases of cerebral infarction patients had high blood pressure and diabetes, that had after onset, were involved in the study, pseudobulbar paralysis. On the basis of neurovascular unit the conception, we gave them Xiao shuan tong granula 3.0·d-1, Isosorbide oral liquid 60ml 3·d-1 , Thioctic acid injection 0.6 1·d-1（ switching to lipoic acid capsules 0.6 2·d-1 after removing the stomach tube） and addind with Perilla oil soft capsule 3.0·d-1 in 2 weeks, after onset. When to with stomath tube, when to resume general fed and the follow up visits after one erar were observed.Results Both as the two patients with drew stomach tubes after two weeks and resumed geaeral fed in 1 month,without adverse reactions.Duning a follow-up of one year there was nelapse and they could livd on their own.Conclusion The custom solution on the basis of NVU in treating patitents with pseudobulbar paralysis after cerebral infarction is reasonable and effective, but more in clinical research necessary confirm.
