摘 要:目的探讨垂体腺瘤病人视野缺损的相关因素。方法总结85例垂体腺瘤病人的MRI及Octopus101视野计检查结果 ,分析视野正常组与视野异常组之间在性别、年龄、病程、肿瘤类型、肿瘤体积及肿瘤侵袭度等间的差异相关性,并探讨垂体腺瘤所致视野缺损类型。结果视野正常27例(31.76%);视野异常58例(68.24%),其中16例为双颞侧视野缺损。视野异常组与视野正常组的肿瘤体积、鞍上侵袭度及鞍上肿瘤扩展高度之间有统计学差异(均P=0.000)。视野异常组肿瘤的鞍上扩展高度,在MRI矢状面和冠状切面的平均值分别为(14.37±6.01)mm和(14.58±5.82)mm,双眼视野平均缺损值(MD)为(6.71±5.55)d B,肿瘤的鞍上扩展高度与病人双眼MD呈正相关。结论垂体腺瘤有无视野缺损及程度与肿瘤体积、鞍上侵袭度及鞍上肿瘤扩展高度有一定相关性,术前分析垂体腺瘤MRI影像学特点可为视野缺损评估提供依据。Objective To explore the factors related to visual field defect in patients with pituitary adenomas. Methods The results of MRI and Octopus101 perimetry test of 85 patients with pituitary adenoma were summarized. The correlation of differences in sex, age,disease duration, pathological pattern, tumor size and degree of tumor invasion were analyzed between normal visual field group and visual field defect group. The types of visual field defect caused by pituitary adenoma were explored. Results The visual field was normal in 27 patients(31.76%) and visual field defect seen in 58(68.24%). The bilateral temporal visual field defect was found in 16 patients. There were significant statistical differences in the tumor volume, invasion of suprasellar space and suprasellar extension height of the tumor between normal visual field group and visual field defect group(all P = 0.000). The mean suprasellar height in MRI sagittal and coronal plane were(14.37 ± 6.01) mm and(14.58 ± 5.82) mm, and the average of binocular visual field mean defect(MD) was(6.71 ± 5.55) d B, presenting a positive correlation with the suprasellar height in visual field defect group. Conclusions Visual field defect or not and the defect content are of certain correlation to the pituitary adenomas volume, invasion of suprasellar space and suprasellar extension height of the tumors. Preoperative analysis of MRI imaging features of pituitary adenomas can provide the basis for estimating the visual field defects.
